The underfunding and privatisation of prisons such as Glen Parva is yet another important reason to question how Government have had some influence over the management and efficiency of the prison.

Since the inquest into Glen Parva, staff have been given further training about when to open the ACCT process. This involves logging details and sharing information.

A further professional issue was that GP notes were sent by postal services. This could led to issues with missing information, slow service.

The hearing into Mr Lambert's death highlighted a series failings that contributed to the outcome.
"Liam was socially isolated and had no visits or telephone calls with friends or family.
"His needs as a foreign national were identified, but not met. furthermore, risk assessments that were conducted were inadequate due to insufficient use of the information available.
All of these issues project legal issues and regulations that were not met. Prisoners have the right to visits and telephone calls but clearly this wasn't enforced.

Nolan (1995) seven principles highlight the importance of accountability. Nolan places emphasis on individuals accepting responsibility and being accountable for their actions.
This is further supported by Martin et al (2010) who makes a distinction between accountability and responsibility. He suggests that organisations give authority to job holders to carry out responsibilities, whilst holding them accountable for the related outcomes.

Arguably, this puts blame on the Government. A prevalent theme is lack of funding received by Government.

Greg Revell, a prisoner at Glen Parva prison was found hanging from his cell. The inquest to his death revealed that Revells needs were not met. Prison officers failed to acknowledge the potential risk Greg was to himself.
