         <title>1. Self-directed Planning and Involvement (Self-Determination)</title>

         <description><![CDATA[<div>A. <a href="https://www.imdetermined.org/">https://www.imdetermined.org/</a><br>B. I'm Determined is a beautifully composed website that helps students to take control of their path in order to have power over what their own future will become. It provides videos and resources for not only students, but also educators and parents. They are also connected on social media such as Twitter and Facebook which is a great way to to connect with youth and give them more access. A transition planning team can utilize this site by introducing it to the student and giving them the tools they need to take control of their future, especially in their IEP meetings.<br>C. I could use videos displayed on this website as a SLP.  These videos are very inspirational and show the journeys of all different students with all kinds of disabilities and how they over come them. These videos can help motivate my students to overcome their own obstacles. </div>]]></description>

The Zarrow Center is a really great program where not only high school students can receive help and resources, but students who are already attending post-secondary education. The Zarrow Center encourages and prepares students to become involved in leadership roles in many different situations including schools and organizations. These leadership skills can also be translated into their IEP meetings and help them take control of their future planning. A transition planning team could use this site and program with their students by showing them an example of what types of programs and resources would be available to them after high school and how this can help them continue to succeed.<br>C. This website includes a Transition Assessment and Goal Generator (TAGG). As an SLP, I can utilize this assessment to push my students and help them find things that they are interested in and what they might want to do with their lives. As an SLP I not only help them with their speech, but also with how speech can help them succeed. With older students I could practice interviews for jobs they are interested in and help with their speech.</div>]]></description>

National Technical Assistance Center on Transition focuses on improving the outcomes for students after high school. It has a bountiful amount of resources for all kinds of topics that SWD may be curious about or be seeking information on such as transition planning, graduation, and post-school success. This website also includes toolkits that these students can utilize for guidance throughout their transition journey. These toolkits can really give students what they need to take the reigns on the transition planning journey. The transition planning team can also use this website to provide very useful resources for their students.&nbsp;<br>C. As an SLP, I would like to be involved in encouraging my students to explore their vocational interests and this website provides many resources that I could utilize in order to do that. The website posts current events and news that could get my students interested in planning their career.&nbsp;</div>]]></description>

         <title>2. Engaging Families During Transition</title>

The Center for Parent Information and Resources has a goal to serve and provide support to families who have children with disabilities. They provide information and materials for topics related to parent center topics, private workspaces for exchanging resources, high priority discussions, and solving challenges, parent training efforts, and a bi-monthly e-newsletter. The transition planning team can offer this site as a very valuable source of support and resources for their parents.&nbsp;This site is super family friendly and would be a great step to getting families involved in the transition planning process.<br>C. As an SLP I also want to reach out to parents and make sure they are involved in their students success. I can use this to help my parents receive support and hopefully encourage them to become more involved in their child's success not only post-secondary education, but with their speech as well.</div>]]></description>

Parents Helping Parents is an amazing organization that helps bring families together who have similar struggles in order to support each other. They do this through support groups. This is great because it is very important to have people who are going through similar situations to lean on, talk to, and share experiences and advice. Transition planning team could definitely introduce this organization to their parents in order to get them involved in the community that they are apart of and help them receive a unique kind of support they cannot receive anywhere else. This support can also potentially get parents to engage more in IEP meetings and transition planning.<br>C. As I previously stated, as an SLP one of my main goals is to make sure that the parents of my students are actively supportive in helping their child in their speech and succeed in life. This is a great support system that I could offer to my parents who might be struggling with being involved and supporting their child.</div>]]></description>

Beach Center on Disability provides many resources and research to help families that include a member with disabilities. One unique part of this website that caught my eye was the Family Employment Awareness Training. This helps individuals with disabilities and their families gain more knowledge on the expectations, competitiveness, and awareness on employment. This is super important because many families do not realize the opportunities or the expectations of employment for individuals with disabilities. The transition planning team could use this to inform their parents of this and how they can become more knowledgable on employment for their child.<br>C. As an SLP I can use many of the toolkits and resources to help get my families involved with my students success in their speech. </div>]]></description>

The first thing you see when you arrive at this website is the question "What do you want to do for a living?". This is great for the transition planning team because the student can immediately figure out where they are in their preparedness for their post-secondary education plan. The site follows with more statements they can choose from including: "I want to be a...", "I'll know it when I see it.", and "I'm not really sure." This is a great way to get straight to the point when it comes to planning a students future and not get overwhelmed with a lot of other information.&nbsp;Transition assessment is super important to getting the student involved in their future planning.<br>C. As an SLP I can use this to gage where a student is headed in their future planning and how they will be using their speech in that future.&nbsp;</div>]]></description>

Similar to my last source, I like how it gets straight to the point of aiding students in finding out where they are in their transition planning and how to further pursue this planning. It provides guidance to students who may not exactly know what they want to do.&nbsp;Students need transition assessments like these in order to gage their interests and point them in the right direction and this assessment is a great tool for that.<br>C. SLPs can use this assessment/generator to encourage students to become excited about what they want to do in their future and begin thinking about how their speech will be involved and what they can do to focus on speech in their future career.</div>]]></description>

This website also provides an assessment but is different in the fact that it focusses on independent living skills. This is so important because SWD need to have the knowledge and a well understood plan for how their living situation will look like outside of high school. A lot of these resources and websites seem to be more focused on the career and education aspect rather than independent living. This is a great website for transition planning teams because of it's focus on independent living skills.<br>C. Having students with IEPs in my future as an SLP I will be involved with IEP meetings and it will be useful to have this resource to contribute to that aspect of individual living skills to the meeting.&nbsp;</div>]]></description>

The first thing you see when you arrive at this website is that this rehabilitation research and training center is involved with many projects that involve individuals with disabilities such as Autism Center for Excellence and Business Connections. Many of these projects include vocational aspects that can help these students with transition planning. The transition planning team could get their students involved in these projects in order to gain experiences and connections for employment which will be super important and useful for when the student is actively looking for work after high school.&nbsp;<br>C. This website provides webcasts in which my speech students could watch in order to gain knowledge on vocational skills as well as watch a model of speech in a professional work environment.</div>]]></description>

The Transition Coalition provides many training resources as well as transition tools. This is very useful to both students and teachers involved in the transition planning team. There are training webinars, ask the expert, and study guides as well as assessment reviews and transition tips.&nbsp;Having all of these tools at the student's fingertips will become very useful when preparing for employment and career development. <br>C. This website has a section that contains updated and current news. In this section there are tons of resources that I could use as an SLP such as a webinar on how to increase grad rates and decrease drop out rates and free lesson plans for transition. These resources are great for me to stay on top of the newest teaching strategies and how to prepare my students for the future</div>]]></description>

This website is a career planning platform that uses videos to guide students through finding their interests in order to pursue their dreams. This website is unique because of its use of videos. I think this is interesting as well as useful because many times students are more likely to be engaged by videos than many other platforms. Transition planning teams can use this to maintain student's interest in the transition planning process and hopefully allow them to take the lead in their career decisions and taking strides in preparing for future employment.<br>C. As a future SLP, I found this website unique and very useful for two reasons. The first reason is because videos are a great way for students to listen and hear how sounds and words should be spoken. The second reason I really liked this website is because it has resources and planning for every stage of development. Most of the resources and websites focused on transition planning are only thinking about high school and maybe middle school students. This website provides options for elementary, middle/high school, higher education, as well as everyone else who may be involved in the planning process. I am planning on working in an elementary school, so I appreciate that their is a section specifically for students in that age range.&nbsp;</div>]]></description>

This website would greatly benefit any person who is a part of the transition planning team for a student with autism. This website provides online learning modules with high-quality information and professional development. It guides users through things like case studies, videos, and pre and post assessments. This is not only good for the transition planning team, but for parents, friends, teachers, anyone who interacts with the student regularly, and it is free! These learning modules will also help the transition planning team figure out the best plan for independent living and inclusion in the community for each individual student.<br>C. In my future as an SLP I will have many students that have autism that will come to me for speech. I would definitely want to use this website to learn how to best work with these students in order to help them in reaching their highest potential and improving their speech more successfully. Students with autism are all so different, so learning more about them and how to work with them would be very beneficial to my career.</div>]]></description>

Although I have already discussed this website, for this section I searched independent living and found so many useful resources to help students and transition planning teams in preparing for independent living as well as involvement in the community. One resource that popped up when I searched was an Independent Living Postsecondary Goal IEP Team Decision Assistance Form. This is a checklist to gage the student's readiness to live on their own and help the transition planning team see where the student needs to improve and what areas the student is already ready.<br>C. My plan in my career in Speech Pathology is to work in an elementary school in which I will not be aiding students in preparing for independent living. This being said, I could change my mind and potentially work in a middle or high school. If so, I would definitely use this as a resource to help my students as well as recommend it to their IEP and transition planning team.</div>]]></description>

The items that stood out to me on this page were the ME! Lessons for Teaching Self-Awareness and Self-Advocacy and ME! Transition Bell Ringers. These two items encourage the student to learn and understand their disability and abilities, rights and responsibilities, and self-advocacy skills as well as self awareness and goal setting. These are all super important aspects when it comes to the transition planning team helping the student prepare for independent living after high school. Students need to know their abilities/disabilities as well as their rights if they are going to live on their own in the future.&nbsp;<br>C. I like the idea of the bell ringer to use as an activity for my speech students. The bell ringers focus on the areas of self-awareness, disability awareness, goal setting, and students gaining the knowledge to lead their IEPs. I could adjust it to be more speech oriented and have students become very aware of their abilities and disabilities when it comes to speech as well as their goals to overcome their disabilities. </div>]]></description>

         <title>6. Preparing for Interpersonal Engagement</title>

This website is super youth-friendly with lots of colors and appealing cartoons. It would be very simple for a student to use. Many students with disabilities need extra help when it comes to interpersonal engagement and social skills. This website offers many things from social behavior mapping to dating tips from the autism community. It is a great site that can be utilized by the transition planning team to have students navigate and learn more about social situations.<br>C. In the career of Speech Pathology and working with students who have issues with speech, their communication and social skills will be a huge part of what I want to work on with them. I want my students to overcome their speech struggles and be able to communicate effectively and this site can help them when they are not feeling super confident about their speech and communication skills.</div>]]></description>

Model Me Kids is a site targeted towards students on the autism spectrum. It gears towards helping these kids in the social aspect of their lives. It includes social skills training and real peer models. Transition planning teams can utilize this site because it models social skills for these students through videos. Showing a child these types of skills through visual motion media allows the student to actually see how these social interactions should look like.<br>C. In Speech Pathology, I love the idea of using videos to model speech, especially videos that will model social situations. I could use these not only for my students with autism, but all of my speech students. Students who have problems with their speech often have difficulties in communication skills and social situations so these videos could help them learn and build confidence.</div>]]></description>

This website focused a lot on interpersonal relationships between adults and children. This is super important for students and teachers/parents to learn and practice when working together. They provide vital, accessible resources and programs to caregivers of behaviorally challenging kids and promote parenting and disciplinary practices that foster the better side of human nature in all children. These are really good resources to improve relationships between children and caregivers and transition planning teams can use this to improve relationships and ultimately the overall goal of future planning for the student.<br>C. As an SLP I find it very important for my parents to have a good relationship with their kids. Because I only get to see my students a few times a week, it is important that I am able to rely on the parents of my students to help them practice when they are outside of school in order to make improvements. A resource like this could help me in being able to do this with my students and parents.</div>]]></description>
