The extravascular implantable cardioverter defibrillator (EV ICD) is composed of two parts device that is implanted outside of the heart and veins below the breastbone and the defibrillator that is placed below the left armpit (Medtronic, 2024). For people living with abnormal heart rhythms or at risk for sudden cardiac arrest, this device is an option to help treat these conditions.

This is longer compared to the average battery life for the subcutaneous implantable cardioverter defibrillator which is 7.3 years (Knight et al. 2023).

Since the EV ICD is implanted outside the vascular space it avoids intravascular complications (Wong & Torbey, 2025).

The EV ICD is capable of antitachycardia pacing or delivering a series of paced beats to help create a normal heart rhythm instead of shocking the patient (Wong & Torbey, 2025).

Opinion and Rationale

In my opinion, the EV ICD is a great step in improving outcomes for patients with heart arrhythmias and at risk for cardiac arrest. With low complications during implantation, it can give patients peace of mind and get back to their daily lives. According to research, there is a 96.9% implant success rate, 99% of patients with an EV ICD had effective defibrillation, and an only 3.9% of patients experience major complications from this device (Medtronic, 2025). I think this device can be improved to reduce the cons mentioned but, overall, this small device has many capabilities to reduce unwanted heart issues.
