Mostly by her behaviour and the way she's communicating. Alongside the pattern, regularity and strength of contractions; a woman's behaviour is the most accurate indication of how her labour is progressing. This can be challenging if we do not know the woman personally, as we may not be aware of what's most confronting for her or what her fears may be and how she felt about birth prior. Generally, the more inward a woman becomes and less able to converse is a good sign of active labour.

Early on in labour a woman may be able to talk during contractions. As they become more intense she might only talk between them. And contractions can be externally palpated, for timing, length, strength and tone.

The vocalisations that a woman makes often change during the course of the labour from crying out sounds to more grunting noises. As the baby's head descends past the bowels they often empty. And transition is often a time when women say that they can't go on, or want some pharmacological form of pain relief, or want to go home.

Assessing a woman in labour involves monitoring physical and emotional changes to determine the progress of labour and identify any potential complications. This includes monitoring the frequency and intensity of contractions, assessing cervical dilation, monitoring fetal heart rate, and assessing maternal vital signs. Also evaluating the woman's pain level and emotional state to provide comfort and support.
