This form should be used for those who handle testing materials from the STC and are in a testing room. This form is NOT required for visitors, subs, paras (all of whom should not be IN testing rooms DURING testing)

Form should be used for NEW staff who also have 60 credits or more.

We have collected Sec Agreements for all district personnel under ACCESS.

Irregularity Form

After reviewing the IEP; any student with this accommodation will need a room with 1:1 scribe. The student can dictate in one device which then must be shared and printed and typed into the text box by the SCRIBE or the student can dictate directly into a 3rd party device after approved by IT.

This form needs to be completed and regardless of either approach, a proctor should be near the room where 1:1 scribing occurs to validate the test is copied directly by the scribe per the student.
