4. Follow the instructions and complete the required information, including your personal details, requested action and the nature of your circumstances.

5. Upload the relevant supporting documentation, such as a medical certificate, where applicable.

6. Submit your application.            

It is advisable to submit your application as close to the time of the event as possible.
