4. Inform residents and families AS SOON AS you become aware that evacuation MAY be occurring AND at the point that ANY evacuation decision is made; Make sure there is time for any alternate planning they may wish to make
5. For any evacuation period assure residents and their families receive updates on your status throughout - be sure that they have a point of contact and number with alternates in different locations so they are always able to reach someone

1. planned evacuation vs shelter in place decision
2. arrival at alternate site if evacuating along with specific of residents and staff present
3. any change in evacuation plan/location
4. any need to evacuate post emergency if initially sheltering in place
5. any damage sustained
6. plans for re-entry if evacuation occurred [NOTE: Health Standards has to approve re-entry and addition steps may be needed if damages/repairs had to occur]
OCDD requires reporting by Support Coordinators daily through LASRS during any emergency situation and to CEA Monitors for any ICF operating CEA services.
