2.     Confirm appointment time – if early, ask recipient to remain in car until appointment time

3.     Take temperature and ensure patient isn’t feeling ill (fever, cough, difficulty breathing, aches)

4.     Provide mask if not already in place and ensure mask is on

5.     Ask if assistance needed and escort to reception area via wheelchair or golf cart if needed

2.     Confirm appointment.  If no appointment, direct to website (linked below) for future appointment options OR collect name, age, and telephone number if no internet access (use notebook provided)

3.     Check identification

4.     If form is completed, check briefly for completion (see separate document) CONFIRM AGE, BIRTH YEAR 1956 OR EARLIER

5.     If form is not completed, provide form on sanitized clipboard, and provide sanitized pen

6.     Provide instructions for completion (see separate document) – direct recipient to picnic table away from reception desk to complete and ask them to return to desk once form is filled out

7.     Provide vaccine information sheet

8.     Instruct recipient to follow flow coordinator to vaccine area

2.     Escort recipient to vaccine area and seat them at open table 

3.     Collect prefilled vaccine from the medication room and carry it in provided container

4.     Tables will flip a card from green to red when they are out of vaccine, after they’ve done so give them 3 more doses

5.     Note how many doses are on the tables in circulation when returning to the med room for more vaccine and tell the staff there so they know how much more to draw up

2.     Review questions and call for site supervisor if questionable for vaccine

3.     Ensure that patient has not received any vaccine, including flu shot and COVID vaccine, within the past 14 days. If they have, call for site supervisor

4.     If patient has notated allergies verify whether the reaction necessitated the use of an epi pen. If they have used an epi pen for any allergy, ensure it is noted on the form and that the flow coordinator is aware as the patient will stay in observation for 30 minutes.

5.     Print, sign, and date consent form and circle title. If information is not pre-filled, add lot number and expiration date per the example.

6.     Complete Vaccine Card with patient name, date of birth, vaccine type, lot number, date of first dose, location- CFK, and return date (back of card)

7.     Inform patient of type of vaccine

8.     Inform patient of return date for second dose

9.     Ask arm preference, note arm preference on consent form

10.  If hx of anaphylactic reaction, ask patient to remain for 30 minutes.  If no hx of reaction, inform patient of 15-minute wait post vaccine.

11.  Don gloves

12.  Cleanse site

13.  Administer vaccine

14.  Apply band aid 

15.  Hand consent form, vaccine card, & information sheet to patient and send them to observation area (flow coordinator will escort)

16.  Vaccine Flip Cards will be provided (red on one side and green on the other).  When out of doses, flip card to red and additional doses will be provided. 

2.     Escort patient to observation area check-in reminding them that they need to stay 15 minutes

3.     If vaccinator informs you that the patient uses an epi pen, inform the check-in counter 

4.     Assist with Flow Coordinator A when they are retrieving vaccine

2.     Collect consent form and verify completion paying particular attention to ethnicity, first or second dose, injection site, vaccinator signature and title, lot number, and patient consent signature. If patient handwriting is ineligible, print patient name on form with appropriate spelling.

3.     Remind patient of return date and tell them to bring card on return date

4.     Seat patient and ask them to remain for 15 minutes (30 if hx of anaphylactic reaction)

5.     Explain the enrollment process and assist with QR code and data entry if needed.  This is optional but encouraged.  Explain that this is the way the CDC will track any adverse outcomes.

a.     For VSAFE website: On an iphone, unlock the phone and point the camera at the QR code and a website link will automatically appear on the top of the screen. Tap the banner and you will be directed to the CDC’s VSAFE website. 

b.     On a smartphone, go to vsafe.cdc.gov

c.      Patient fills in name, date of birth and phone number and will then be texted a registration link. They must use the link to verify their phone number in order to fully register. They will then receive a daily text check in for the first week and a weekly text check in thereafter as well as a reminder for their second dose. 

d.     They do not need to reregister for their second dose.

6.     Circulate periodically to make sure no one is feeling ill.  If symptoms of adverse reaction are present (short of breath, rash, pain, headache, chest pain, dizziness, or any new complaints), call for site supervisor.

7.     When 15/30 minutes have passed, patient is free to go.  No need to check-out.

b.     On a smartphone, go to vsafe.cdc.gov

c.      Patient fills in name, date of birth and phone number and will then be texted a registration link. They must use the link to verify their phone number in order to fully register. They will then receive a daily text check in for the first week and a weekly text check in thereafter as well as a reminder for their second dose. 

d.     They do not need to reregister for their second dose.

2.     Label each syringe with the time the dose expires, 6 hours from when the first dose was removed from the vial

3.     When the vial is empty dispose in the sharps container

4.     Do not open a new vial if the number of doses in circulation exceeds the remaining number of appointments towards the end of the day. Do not open a new vial if there are less than 6 doses needed. Contact site supervisor with questions

2.     Before personnel go to their designated areas ensure that everyone knows the date, the lot number in circulation, expiration for that lot number, and the return date. Ensure that everyone knows to notate CFK on the vaccine cards for vaccine location.

3.     Ensure that all volunteers are familiar with the paperwork they will be responsible for and that an example form is available to them.

4.     Be aware of the 6-hour expiration on the syringes in circulation to ensure that doses nearing the end of their window are used first.

5.     Patients who received another vaccine within the past 14 days cannot receive the vaccine, patients should be told when the date is that they can reschedule their appointment.

6.      Patient’s showing active signs of illness, especially COVID, need to return when they are no longer ill or in two weeks.

7.     Patients who are 64 but turning 65 within the calendar year are eligible under the Governor’s order to receive the vaccine, anything younger is ineligible unless they also meet other criteria.

8.     Towards the end of the scheduled appointments coordinate with the medication room staff to determine how many doses are still in circulation with respect to how many remaining doses are needed to determine if and how many remaining vials should be opened.
