2. Your progress so far

 

This week I got an opportunity to deal with the real patient. It is not difficult enough to handle it with guidance. Patient was so nice because he knew that what kind of problem that he had. He knew when it was started and the causes. This week I learned from Kak Wani how to use audiometer to check the hearing severity of the patient. This was new to me because I never had or use it before but with Kak Wani’s guide everything went well.

 

3. Your feelings and thoughts 

 

It was very challenging for me because I never use audiometer before. At first, I thought that when the first time I saw Kak Wani use the audiometer it was very easy to handle it but when she asked me to do in front of her everything was disaster. She just asked me to do the test to her not the real patient. 

 

4. How did you learn and apply the skill of decision making in your EP journey?

 

I learned that the effective skill of decision making is by listening effectively to the instruction as what as audiologist told me to do so. It is because without listening properly, everything will not be done smoothly. Moreover, people in surrounding will be more comfortable to talk with because if we can really understand what we have been told, they can teach new things because they no need to repeat the same things again. By being a good listener can make a day with full of joy. 

 

5. How did you manage your time throughout this EP journey? 

 

I made a schedule because works and my own activities need to be balance. My workplace is not far enough but to go there it was very tired because along the way the road was filled with many kind of transportation. It was very stressful but with the right schedule I made, I made it through the journey. 

 

6. Your plan for next week (Week 4)

 

My plan for next week I want to learn more about the things I never had before.

2. Your progress so far

In the third week as a clinic assistant, I help the GP to do wound dressing and debridement. The wound debridement usually perform on diabetic patient with diabetic foot ulcer and poor wound healing. I am eager to try suturing a wound however there is no such case that need suture for the past 3 weeks. I also learn to prepare the mixture for nebuliser (normal saline + salbutamol). The nebuliser is used in patient with AEBA with rhonchi and need to repeat if the rhonchi is still heard after the first nebuliser was given. I also learn the various type of cough medication that suitable according to the patient presentation for example in patient with asthma there is cough medication that contain salbutamol and in diabetic patient that don’t contain sugar. 

 

3. Your feelings and thoughts 

This is my first time doing wound dressing and debridement as we just observe the staff nurse doing it in the ward. When doing the wound debridement, I always ask the patient if there is any pain whenever I debride the deep wound and when the blood come out because I afraid if I am causing to much pain during the debridement process, however most of the patient does not perceives the pain as they have diabetic neuropathy. 

 

4. How did you learn and apply the skill of decision making in your EP journey?

URTI is the most common presentation that we see every day in the GP clinic. When deciding to prescribe antibiotic in case of URTI, the GP thought me that in patient with acute tonsilitis and pharyngitis are usually cause by bacteria and need antibiotic. Centor score is also used in making the decision.  

 

5. How did you manage your time throughout this EP journey? 

Throughout this EP, I sleep on time and woke up early especially for morning shift. At work I will organise my task and delegate the task between my other co worker as there is only 3 clinic assistant. 

 

6. Your plan for next week (Week 4)

For next week I plan to perform venepuncture, IM injection and set an IV line. I have observed those procedures before and would like to try performing them on real patient. 

I plan to finish studying about the medication available in the clinic and gain experience by performing venipuncture if I got the chance to.

2. Your progress so far

Thus far, I have managed to learn about the management and processing of air ticketing, hotel booking, and tourism overseas. I was given the responsibility of exploring the possibilities of the company's development in outbound tourism and being directly involved in researching a new suitable lodging package for Turkey offered by the company. By the end of the week, I had pitched my report to my team, and we are currently working to realise the vision.

3. Your feelings and thoughts 

I am greatly honoured that the company has entrusted such an important task to me, and I am very proud of myself for being able to handle it well. I am pleased that my efforts and constant worries paid off. I have gained more confidence in problem solving and pitching ideas, which are two vital components of my future, especially in class.

 

4. How did you learn and apply the skill of decision making in your EP journey?

In order to make a decision, I had to weigh my options. For instance, in choosing the most suitable hotel for lodging, I have to consider a number of factors, including strategic location, affordability, accessibility, comfort, and facilities. Among many options in Cappadocia, I chose an affordable hotel with a beautiful, serene view from the terraces overlooking the main attraction (hot air balloons), breakfast included, and accessibility to nearby restaurants, bus stops, and supermarkets.

 

5. How did you manage your time throughout this EP journey? 

I found it quite difficult to maintain my working hours between 9 a.m. and 5 p.m.I spend my day doing odd jobs in addition to the main task assigned to me. So, I used a planner to keep track of my responsibilities. However, I prioritised self-care and work-life balance, in which I made sure to eat on time, spend my evenings with family, and have sufficient sleep. Therefore, I opine that I have done well in managing my time.

 

6. Your plan for next week (Week 4)

In the following week, I will be expected to finalise the projects and meet the customers upfront.

2. Your progress so far

This week, I got the opportunity to observe the cupping treatment. I learned the function of cupping treatment and make comparisons between cupping and acupuncture treatments. 

 

3. Your feelings and thoughts 

Cupping treatment is not that frequently used compared to acupuncture treatment in the clinic. Local suctions are created on the back with the application of heated cups. Then, the Chinese traditional medication practitioner will move the cups to other part of the back until entire part of the back is fully covered with red marks. It is an interesting and new thing for me when I saw the process of the treatment.  

 

4. How did you learn and apply the skill of decision making in your EP journey?

As I done my EP in an acupuncture centre, I faced patients same as in hospital. After history taking and examining the pulse of the patient, the Chinese medication practitioner will decide the most appropriate treatment for the patient. In this situation, it is important to apply the skill of decision making. A good decision making can give the patient with the best outcome and avoid unnecessary expenses. 

 

5. How did you manage your time throughout this EP journey? 

It is difficult to manage my time throughout this EP because I need plenty of time to study the new concepts of Chinese traditional medication. Therefore, I manage my time by setting targets that I would like to achieve for the day. It would be easy to manage my time with this planning method. 

 

6. Your plan for next week (Week 4)

I plan to learn some herbal medication and if possible, I hope that could observe the acupuncture or cupping treatment.

Finally it came to the D-day.

One of the highlights of the Women First Campaign was that we would be taking military plane together with the soldiers to our destination. Stepping into the military aircraft, I was immediately struck by the aura of controlled chaos that surrounded me. The interior was dimly lit, illuminated primarily by the soft glow of instrument panels and red warning lights. The low hum of engines reverberated through the metal walls, creating an atmosphere that seemed straight out of a Hollywood movie. As I settled into my seat, the vibrations beneath me intensified, accompanied by a subtle rumble that seemed to resonate with the beating of my own heart. 

We took a 2 hours military plane from TUDM Subang to Kuching Airport, then another 2 hours transit to Kota Kinabalu Airport, then took a 10 hours shuttle bus to Semporna Jetty and finally took an hour boat to Pulau Kalapuan. 

When we first arrived, I remembered how the villagers welcomed us with big wide smiles and the children there even offered to carry our baggage and medical equipment. We spent the first day setting up all the stations in the daytime, then play bonding games with the people in the evening. We divided the campaign into a few components: community clinics, basic life support courses, and women first training. 

For community clinics, we did all the basic clerking, anthropometric measurement and vital sign recording. For first aid courses, we teach the community healthcare workers about CPR, choking, jellyfish stings and so on. For women first training, we teach them about safe home deliveries and proper breastfeeding techniques. All programmes are structured considering the geographical hindrance which complicates their quest for medical attention, especially in cases of obstetric emergencies during the home deliveries. While most know Malay, many still do not fully understand Malay and speak in native languages like Bajau and Tausug. The thrust of this rural outreach is to prioritize primary prevention measures over secondary and tertiary options. Specifically, the initiative seeks to underscore the need for robust interventions geared towards tackling sexually transmitted diseases, promoting family planning, and ensuring capacity-building for expectant mothers.

I feel heartbroken to see that many families experience child deaths under five and many children are underweight and malnourished. For example, most of the families have at least 10 children however two or three children passed away under five. Most of the 15 year old childrens’ height is only 120cm.The predicament of geographical hindrance exacerbates the  stateless women in the community who try to seek medical treatment, and would oftentimes be imposed with elevated charges due to their non-citizenship status. 

Decision-making is assimilated throughout the EP journey as we face challenges ranging from linguistic and literacy disparities to exhibiting cultural sensitivities and prevailing gender dynamics. Therefore, we have to come out with our own teaching materials catering to the community’s literacy level. Through tailored strategies, open dialogue, and community engagement, we addressed these difficulties and improved Sexual and Reproductive Health outcomes.

Apart from that, the community feels quite intimidating when we first visit for population check as they categorized us as the so-called city people who came to chase them off the island. This is also when we made up our mind to collaborate with a NGO that has been there earlier than us, ‘SEDULUR’ as this NGO has been assisting the community to set up a water system on the island. To get into the field effectively, we acknowledge that the bonding with the community is important. Thankfully, the collaboration plan works surprisingly good!

Every week is occupied with impactful activities and this makes me feel time flies. I would say my time management is good. I have allocated fair enough time for meetings as well as time for myself to discover more of Semporna.
