Use this spreadsheet to find out what students need to be entered into MCIR for immunizations. All new students since the last MCIR deadline must be on the roster for current listing.

Several of our documents needs this rate to be included.

This is also the reason we need a signed Exit form or a Records Request from parents to verify that a student is leaving BHHS. The State of Michigan requires verification.

Anytime you know of a Senior, ARP, or FRP student leaving BHHS or not graduating, please enter them on this sheet and notify Laurie Mezey of the change.

Create a new spreadsheet at the end of 1st semester each year to begin the process for the next year.

This sheet is shared with counselors, administration, Student Services manager (Tawn F), and Virtual Enrollment teacher (Ebony D)

These students turn in a Pass/Fail form to you and this is where you keep track of all the forms turned in.

If you can figure out how to collect the parent and teacher signatures, I suggest a google sheet would be more efficient. I didn't have time to figure out how to verify those signatures.

Grade is entered as semester T with a grade of T on the date the test was given.
