         <description><![CDATA[<p><strong>Production of B-lactamase enzymes</strong></p><p>These enzymes break down the antibiotic and destroys it and the antibiotic is no longer effective to treat an infection. Examples of these types of antibiotics include penicillins and cephalosporins</p><p>&nbsp;</p><p><strong>Lowers the permeability of the membrane</strong> so the antibiotic cannot enter the cell</p><p><br></p><p><strong>Forms a protect barrier (capsule and biofilm)</strong></p><p><br></p><p><strong>Employment of efflux pumps</strong> to remove antibiotics from the cell as a defense.</p><p><br></p><p><strong>Enzymatic modification of the antibiotic</strong> to make them ineffective<strong> </strong></p><p><br></p><p><strong>Transfer plasmids and transposons</strong>  This mechansism of resistance is the creation of ESBLs (plasmid-borne extended-spectrum B-lactamese). ESBL are genes that pass on the code on mobile plasmids that will enturn code the genes for resistance.          <title>Current treatment challenges</title>

         <description><![CDATA[<ul><li><p>Multidrug resistant E. coli strains are becoming a major challenge in treatment options for patients and these strains of E. coli are increasing all over the world. One of the most common gram-negative bacterial pathogens that cause disease and infection is caused by E. coli. An example of the rise of multidrug resistant E. coli strains is a documented increase of 300% more hospital admissions of patients that was linked to ESBL producing E. coli.</p><p><br></p></li><li><p>Antibiotic resistance to common antibiotics that are used to treat UTIs caused by E. coli are trimethoprim-sulfamethoxazole, ampicillin, and fluoroquinolones. Due to E. coli strains that produce B-lactamase enzymes, the most common class of antibiotics (cephalosporins) that are commonly used to treat gram-negative bacterial infections, are no longer effective. Just between 2000 to 2009 has hospital admissions caused by multidrug resistant and ESBL producing E. coli has now increased by 50%</p></li></ul><p><br></p><ul><li><p>Now being reported all over the world are not only multidrug resistant strains but also extensively drug resistant and pan-drug resistant strains.          <title>Identification and Characteristics of E. Coli </title>

         <description><![CDATA[<p><strong>Identification</strong> <strong>of E. Coli</strong></p><p>Also known as (<em>Escherichia coli</em>)</p><p><em>Escherichia Coli </em>involves several steps and techniques when comes to microbiology. A few methods to identify E. coli are mentioned below.</p><ol><li><p><strong>Sample Collection and Culturing</strong></p><p>-samples are collected from sources such as water, food or specimens. The samples are collected in a selective media and inhibits gram-positive bacteria which allows gram-negative bacteria to grow e.coli and appears to stain as pink or red.</p></li><li><p><strong>Gram Staining</strong></p><p>-Small samples of the cultured bacteria is stained using the gram staining technique.</p></li><li><p><strong>Biochemical Test</strong></p><p>-Indole test, Methyl red test</p></li><li><p><strong>Molecular Techniques </strong></p></li><li><p><strong>Serotyping</strong></p><p>-it can be tested with surface antigens.</p></li></ol><p>These combinations can all help and identify E. coli samples.</p><p><br></p><p><strong>Characteristics of E.coli</strong></p><p><em>Escherichia coli</em> has a diverse of groups with bacteria.          <title>E. coli Pathogenicity</title>

         <description><![CDATA[<p><em>E. Coli </em>lives in the intestines of people and animals and is mostly harmless because they contribute to intestinal health. It is when eating or drinking food and water that is contaminated with certain types of <em>E.Coli</em>, that can be threatening to someone. E. coli can be pathogenic by toxins, invasion, adhesion, evasion.</p><p><br></p><p><strong>Pathogenic by Toxins: </strong></p><p>Biological poisons are produced by some <em>E.coli</em> strains. These poisons can invade the cells of the intestinal lining and cause diarrhea. These are categorized into, Enterotoxigenic <em>E. coli.</em></p><p><br></p><p><strong>Pathogenic by Invasion: </strong></p><p><em>E. coli</em> can invade the cells lining of the intestines, this disrupts normal cellular functions. These are categorized into, Enteroinvasive <em>E. coli.</em></p><p><br></p><p><strong>Pathogenic by Adhesion: </strong></p><p>Some <em>E. coli</em> strains have hair-like structures called fimbriae. These are proteins that allow them to adhere to the surface of host cells receptors. These are categorized into, Adherent-invasive <em>E. coli</em>.</p><p><br></p><p><strong>Pathogenic by Evasion:</strong></p><p>This is a mechanism that allows E. coli to produce a protective capsule of polysaccharides. This manipulates the host cell and prevents the immune system from recognizing and eliminating the bacteria. This allows them to infect the host.          <title>Epidemiology- Prevention and Control</title>

         <description><![CDATA[<p><br></p><p><strong>Prevention</strong></p><p>E. Coli can be prevented within the majority of the group as long as basic hygiene concepts are followed with an emphasize on “keeping your hands clean, preparing food safely, and drinking safe water”. Although it seems obviously simple, doing these things right is how we protect ourselves and the population of an outbreak. This means washing our hands for at least 20 seconds under clean water and soap, following the four steps for simple food safety(clean, separate, cook, chill), and avoiding drinking any potentially harmful and unclean water like when traveling or in the pool. It’s always best to drink pasteurized milk and juices as well as these liquids can hold this harmful bacteria without going through the process to kill them first. It’s a team effort and staying responsible of what you do and making smart choices, like not going for a swim when experiencing diarrhea, will protect many people by lowering the chances of having a serious outbreak of E. Coli.</p><p><em>-Healthcare Setting-</em></p><p>When taking care of a patient that has E. Coli- preventative measures should always be taken with the patient until the bacteria has completely passed through their system(usually after patient is no longer having diarrhea). Barriers such as gloves and gowns should always be on when handling infected patients (especially with their fecally soiled items), rinsing soiled materials can only be done in designated areas such as a hopper sink and bagged during transportation to laundry, medical equipment used on infected patient should only be used on them until they are no longer infected, and changing of protective barriers (gloves and gowns) as well as thorough handwashing has to be practiced after handling the patient in order to avoid an outbreak within the healthcare setting.</p><p><br></p><p><strong>Notable Outbreaks</strong></p><p>Within the last two years alone the U.S. has experienced four outbreaks of E. Coli that the CDC had to get involved and source where the contaminations had started from. Surprisingly enough, only one item in the list had sourced from a dairy product which was raw cheddar cheese. The rest of the three items in the list that had started an E. Coli outbreak were carrots, onions served on McDonald’s burgers, and organic walnuts. Of course the U.S. isn’t the only country with outbreaks going on as the UK had also experienced two outbreaks during this sane time period, but it does seem that the U.S. has been experiencing more outbreaks of E. Coli compared to other regions.</p><p><br></p><p><strong>Populations at Risk</strong></p><p>Anyone could get infected with E. Coli  which is why everyone is at risk and should take preventative measures at all times to avoid obtaining and spreading it. Although this is true though, there are certain groups of the population who are not only more at risk of getting infected but also have increased chances of receiving the more severe side of the illness. The groups in question are people with weakened immune systems, children under 5, adults over 65,  and travelers. What these groups share in common that makes them vulnerable is that their immune system isn’t as strong as the majority of the population or haven’t been given the right environment nor time to build their defense mechanisms. As it’s harder for these peoples’ bodies to fight off the bacteria, it’s what makes these people more susceptible to E.
