Early in the morning Bethany, her best friend Alana, and Alana’s father and brother decided to go surfing. The four of them were chatting on their surf boards out in the ocean waiting for waves to form. All the sudden out of nowhere a giant shark leaps from the water at Bethany and bites her arm off. Bethany goes white and Alana starts to freak out. The three of them together are able to get to the reef and pull Bethany to shore.

I would want to know the following:

VS, LOC, other injuries, signs of shock present, time of incident, ETA, interventions done on scene (tourniquet or pain meds given), any patient allergies or pertinent past medical history.

In preparation of this patients arrival, I would ensure the trauma bay was warmed and warm IV fluids with a hot line were ready. A massive transfusion protocol would be initiated due to the amount of blood loss and loss of a limb. The trauma team would be activated including the trauma surgeon, anesthesiologist, respiratory therapist, imaging personnel to determine extent of injuries, etc. I would also notify the OR that patient is coming and potentially needs surgical intervention.

The only thing I would add is the patients age and gender. As well as how the patient lost their limb, and which limb.
