Introduction: We already developed a telemedicine system using a cellular phone and internet web site to monitor changes in blood pressure and pulse rate in patients at home (Nakamoto et al., Hypertension 42: 442â€"3, 2003). Using this system, we have conducted the blood pressure control crossover study with amlodipine vs. nifedipine CR. Methods: Patients who were not taking an anti‐hypertensive ca‐antagonist other than amlodipine (A) or nifedipine CR (N) were eligible to enter this study. The patients in the first group (n 11) started A 2.5 mg/day. Doses of A were increased to achieve home blood pressure less than 135 / 85 mmHg to the extent of 7.5 mg/day. If necessary, atenolol was added. After achieving the target blood pressure, home blood pressure was monitored for more than 6 weeks and then A was changed to N. Home blood pressure monitoring was continued for more than 6 weeks after changing medication. The patients in second group (n 9) took N firstly, and then changed to A. The alternative doses were as follows; A 2.5 mg/day N 20 mg/day, A 5.0 mg/day N 40mg/day, A 7.5 mg/day N 60 mg/day. Blood pressure was checked at least twice a day, in the morning within 1 hour after wake‐up and in the evening before going to bed. Data of blood pressure and pulse rate were measured by automatic sphygmomanometer and were directly collected by i‐converter. All data including the time of measurement were directly sent and accumulated in the main server constructed in NTT DoCoMo data center through cellular phone operated by i‐converter. We could download data to CSV file like Excel and analyze them easily. Results: The mean of morning blood pressure in each period of medicine was significantly decreased in N period ( 133.0 1.1 / 78.1 1.1 mmHg (A) vs. 130.0 0.9 / 77.9 1.0 mmHg (N), p 0.05 ). The mean of morning pulse rate significantly increased in N period ( 68.4 1.2 beats / min (A) vs. 70.5 1.3 beats / min (N), p 0.05 ).Conclusion: The blood pressure reduction effect of nifedipine CR is stronger than that of amlodipine. Also, nifedipine CR increases pulse rate at our alternative dose setting. In this study, we have collected more data of home blood pressure in the quality and quantity than the previous studies through our telemedicine system. We also reduced the effort of data analysis with this system. (59th Annual Fall Conference and Scientific Sessions of the Council for High Blood Pressure Research)

The development of telemedicine in the world.

The phenomenon of telemedicine did not arise in our lives due to the coronavirus pandemic, but it was in the last two years that this sector has become more relevant than ever before. Social distance, which is one of the key conditions for the prevention of COVID-19, is the main constituent element in telemedicine since the assessment, diagnosis, and treatment of patients by medical specialists is done remotely using telecommunication technologies. The use of this approach can be traced back to the previous decades; however, it has been sporadic and limited due to the high cost and complexity of the equipment. Initially, telemedicine was most often used for consultations between general practitioners and their more specialized colleagues situated in remote locations, such as medical centers or campuses away from rural or hard-to-reach populations. The situation has changed due to the wide and ubiquitous spread of the Internet. The creation of communication devices capable of providing high-quality communications, both audio and video, has provided tremendous opportunities for remote patient assistance at home, in the workplace, or nursing homes.

In addition to greater security, that is, the absence of possible contact with other potentially infectious people, telemedicine provides several other benefits for patients, including saving time and cost on a commute and procedural processes, a higher level of confidentiality, and the availability of any specialists. Regarding doctors and medical staff, they report improved efficiency in their services and, as a result, increased income, as well as better patient follow-up and improved health outcomes [Chironhealth, n.d.].

The use of technology (computers, video, phone, messaging) and medical software makes it easier and cheaper for patients to manage their health with increasing levels of chronic disease. Patients with such diseases need frequent tests, medical checks, dietary advice, and other emergency care. Enabling patients to use telemedicine for these services eases the burden of a chronic illness and increases the likelihood that the patient will comply with follow-up instructions. The use of a dedicated post-hospital follow-up program for patients with heart failure reduced the number of readmissions within 30 days by 73% [Smith, 2015].

Article is dedicated to research work in telemedicine field including statements required for e-medicine and based on preliminary development results of computerized screening complex, with wireless transmission of measurements to analysis center, computerized decision-making realization with certified expert conformation and feedback of conclusion to user. The developed Computerized Screening Complex is based on two main information sources: Interactive questionnaire to register subjective symptoms in about 12 human organism subsystems; and Modular electronic device for 14 physiological test registration with additional 3 test programs for objective parameter measurement and system testing. System hardware part of Mobile Telemedicine Screening Complex (MTSC) is based on set of specially designed measurement modules. Overall MTSC modules can acquire more than 60 parameter values - numeric, nonnumeric, number strings, or curves, measurement values and calculated values. Computer included in MTSC provides more additional features, result visualization and data flow management. Measuring modules in the unit include separate measuring operations management and recalculation of raw parameter values. Information transmission unit send the results of digitization and preprocessed data to analysis center. Its task is to check the information received, provide detailed analysis, including computerized and expert evaluation of individual tests (like ECG, dermascopy), correction and approval, confirmed by certified specialist at analysis center and storage of tests and results in the database. Consequently link to analysis center is duplex, the indication is provided on both ends of the system.

Tele-education has been used for many years to deliver continuing education programmes to rural health-care professionals. The main modes are audio, video and computer. Audio technologies involve the transmission of the spoken word (voice) between learners and instructors, either synchronously or asynchronously. Examples of the former include audioconferencing and short-wave radio; examples of the latter include audiotape or audiocassette. Video for distance learning, like audio, can be used in either synchronous or asynchronous fashion. Videoconferencing, or interactive television, are considered synchronous because there is the opportunity for live visual and verbal interaction between instructors and learners. Asynchronous instructional video tools include slow-scan video, interactive videodiscs and videotapes. Computer-assisted learning or instruction can be defined as any learning that is mediated by a computer and which requires no direct interaction between the user and a human instructor in order to run. It is becoming increasingly common. Examples include: the Internet and World Wide Web, email, synchronous and asynchronous computer-mediated communication applications and interactive multimedia applications on CD-ROM. Tele-education technologies have an important role to play in addressing the professional isolation which is experienced by rural and remote health-care professionals.

A teleconference is a live audio or audiovisual meeting with two or more participants. With the ability to teleconference, remote teams in an organization can collaborate and communicate, even when geographically dispersed. The process involves technology more sophisticated than a simple two-way phone connection.

At its simplest, a teleconference can be an interactive audio conference with people at two or more locations communicating over a speakerphone. With more equipment and special arrangements, a teleconference also can be a video conference, in which the participants can see each other.

Telecommunication systems support teleconferences by providing audio, video and data services. Participants communicate with teleconference platforms using devices such as desktop computers, tablets, smartphones and laptops. Teleconferences were initially conducted through telephone lines and were limited to audio; however, now it is more common for teleconferences to be conducted online or using voice over IP (VoIP).

Types of teleconferencing

Teams in an organization have the option to conduct teleconferencing through multiple means, including the following:

Audio teleconferencing. These voice-only calls are conducted similar to normal telephone calls but can support up to 100 participants.

Video teleconferencing. This form of teleconferencing combines live visual and audio mediums. Depending on the vendor, video conferencing can support over 100 participants. Users in the meeting can use features such as screen sharing or file sharing.

Web teleconferencing. A web teleconference is an umbrella term that describes teleconference services or mediums provided online, which includes web meetings, webinars and webcasts. A video conference is, therefore, a type of teleconference.

Control of Infectious Illness

To help prevent the spread of COVID-19, flu and other infectious diseases, doctors can use telehealth appointments to prescreen patients for possible infectious disease. It also saves sick people from having to come in to the office. Less exposure to other people’s germs helps everyone, especially those who are chronically ill, pregnant, elderly or immunocompromised.

Better Assessment

Telemedicine can give some specialty practitioners an advantage because they can see you in your home environment. For example, allergists may be able to identify clues in your surroundings that cause allergies. Neurologists and physical and occupational therapists can observe you and assess your ability to navigate and take care of yourself in your home. Telemedicine is also a good way to get mental health assessment and counseling.

Family Connections

When consulting with your doctor, it’s always good to have a family member who can help you provide information, ask questions and take note of your doctor’s answers. If that person lives out of town, or even across the country, telemedicine can loop your family member in on the virtual visit if you authorize it.

Primary Care and Chronic Condition Management

Regular visits with primary care practitioners such as those specializing in family medicine, internal medicine and pediatrics, are essential to your family’s health. Telemedicine makes it easy to connect with a doctor or nurse practitioner. Some systems are set up so that new patients can get an appointment with the next available practitioner, which can save time.

Telemedicine has a long and interesting history that dates back to the 19th century. In the 1860s, during the American Civil War, the telegraph was used to inform medical teams about injured soldiers [2]. After William Einthoven created his first ECG in the early 20th century, information was shared between his lab and academic institutions [3]. Later the telegraph was replaced by radio and telephone which was used primarily for providing ship personnel with healthcare advices [4]. Between 1940 and 1970, there were several successful transmissions of clinical data, both over short distances and across continents [5]. After its invention, the television was also used in medical education. In 1944, Johns Hopkins Hospital broadcasted an operation through the aid of closed-circuit television to a large number of surgeons [5]. Between 1970 and 2000 rapid and substantial revolutions in connectivity and devices led to the initiation of contemporary telemedicine. The phrase “telemedicine” was first proposed by Murphy and Bird [6] in 1974 after they successfully conducted 1000 patient encounters between the medical station of Logan International Airport and the emergency ward of Massachusetts General Hospital [6]. The biggest advancement in connectivity, the availability of the internet to the general population in addition to rapid advancement in technology facilitated telemedicine development [5]. By incorporating virtual hospitals into medical care, telemedicine has been involved with a diverse scope of services including and not limited to neonatology, fetal care, neonatal resuscitation, remote rounding, tele-specialty consultations, and parental education [5]. In 2020 a study on the US telemedicine market stated that at that time more than 75% of United States (US) hospitals had some form of telemedicine program [7]. Nowadays healthcare providers, using live chats and video calling, are providing healthcare directly to patients in their households.

In today’s society, information and communication technologies (ICT) play an important part in almost every area [1, 2]. ICTs have the potential to promote patient-centered healthcare at a reduced cost, increase quality care and information sharing, educate health professionals and patients, stimulate a different sort of interaction with patients and health providers, and minimize travel time [3, 4]. The healthcare system is growing increasingly dependent upon technology, so, health science students are expected to improve their skills of ICT [5, 6]. Also, ICT enables the use of novel educational materials and the renewal of learning methods, allowing students to collaborate more actively and simultaneously acquire technological expertise [7]. In spite of the fact that there’s no single, widespread definition of ICT, the term is for the most part acknowledged to cruel all devices, organizing components, applications and frameworks that combined permit individuals and organizations associated within the computerized world [8].

While we understand that the use of educational technologies in higher education teaching and learning activities is still in its infancy in Ethiopia, ICT instructional use is critical to both faculty and student success and development [9]. In education, the use of ICT to improve or support learning and teaching has grown increasingly significant [10, 11]. For several decades, many authors have argued that ICT as educational devices facilitate the adaptation of teaching to each student [9, 12]. Although it is widely assumed that ICT can empower teachers and students, encourage change, and nurture the development of ‘twenty-first century skills, data to back up these claims is still scarce ( [9], Https://www.infodev.org/innovationandEntrepreneurship). ICT in health care, and both educational opportunities and assistance for students and health care professionals have expanded [13, 14]. The COVID-19 epidemic has had a significant impact on all aspects of schooling, particularly in light of existing social distancing norms. Medical educators are employing a variety of information and communication technology (ICT) methods to maintain medical education in the face of the pandemic. The majority of medical educators use university websites and online collaboration technologies to disseminate study materials [15, 16].

Computer technology and digital world have revolutionized the way people live, work, development and distribution of knowledge and power around the world (Https://learningportal.iiep.unesco.org/en.learning portal,2021). Because the integration of informatics into health science education is becoming necessary in many universities across the world, it is critical to assess health science students’ factors to use computers and ICT technologies [17,18,19]. Expanded usage of eHealth is projected in Ethiopia in the future years, appreciations to new efforts, but these systems must be used properly to accomplish objectives; this is entirely dependent on health science student since they will be professional in few years later [20].
