This is an awfully weird comparison, so I came up with two thoughts:
1) The main character Tally made the comparison, and she's an Ugly, so maybe she hasn't seen a whole lot of stuff, so maybe cat vomit is the only thing she could compare to.
2) Cats could be significantly important to Tally, so that could also be why that comparison was made. 

I feel like the cat vomit may symbolize Tally's life experiences. I also think this because when Tally get's to the Smoke, we can see that people don't need to be pretty to live a normal life.  All you need is to work hard to be the best you can be and you're good!

In conclusion, Tally's and Shay's actions help us to see the theme of the book.
