Social Determinants

The first social determinant that I think the Migrant/Refugee people deal with is "Food insecurity". When migrants arrive in a foreign country, the first basic necessity to support their lives is their food. But due to cultural differences migrants find it difficult to find their food and even if they find their food, the taste of food is not the same. Also, there is always an insecurity about their food when they leave their home country as they always have a thought in mind wheather they will get their food or not.

The second social determinant that migrants face is "Unemployment and job insecurity". Migrant People sometimes face problems with employment due lack of proper certification proofs even after having the required skills. Another reason that migrants may face unemployment is the language barrier. Due to language changes, migrants are not able to communicate to their fullest, which can sometimes lead to unemployment, which ultimately affects their lives.

The third social determinant would be "Access to affordable health services of decent quality". As the majority of migrants are not permanent residents of the country they migrate to, affordable health care could be a challenge for them, because they are generally not entitled to the rebates in health services that the government gives to their residents. And seeking medical services might become diificult for them, due its high costs.("Social determinants of health," 2019)

So these were the Three social determinants that might affect migrants lives.

References

Social determinants of health.
Domain 7 : Professional practice

After going through Domain 7 of Culturally Responsive Clinical Practice: Working with People from Migrant and refugee backgrounds, I gained significant knowledge about how to provide culturally safe care to migrants/refugees. The domain discussed competency standard 12 which further had its sub-parts. As a whole the competency standard discussed the impact of the interaction of health professionals on migrant/refugee health, clinicals should ensure that they talk with dignity and respect to migrants and must take care of their safety in a healthcare setting. Moreover, it also talked about providing flexible care to people from different cultures, which should be appropriate to their culture. Also, clinicians should respect migrants in their healthcare decisions as their culture might influence their thoughts.
So these were some of the learning that I found after reading the domain 7 and during my clinical practice I will try to apply them.
https://moodleprod.murdoch.edu.au/pluginfile.php/3573073/mod_resource/content/1/Culturally-responsive-clinical-practice-Working-with-people-from-migrant-and-refugee-backgrounds-Jan2019.pdf

Country of focus: Afganistan. After the return of the Taliban to power in Afganistan, the locals are facing the world's worst humanitarian crises. More than two-thirds of people face hunger and millions of children are malnutritioned. Moreover, Afghanistan also has the world’s highest infant mortality rates, and thousands of women die from pregnancy related problems, the majority of which can be prevented. After all this scenario the Australian government has been continuously increasing the number of humanitarian visas for locals of Afganistan. Recently in 2023, the Australian government has allocated 26,500 visa spaces for them. Three common health concerns listed on the Australian refugee health practice guide that are likely to be present in people arriving from Afghanistan are:

1. Malaria

If a Migrant tests positive for malaria he/she is generally treated with antibiotic v1576. To be specific if the malaria is caused by P. falciparum then a single dose of primaquine is recommended.

2. Mental health

Many refugees face problems like trauma, family separation and torture which generally leads to mental illness, if not treated. Most of these problems are due to their circumstances in their home country. As a part of treatment, migrants are referred to some screenings such as Trauma screening. Patients with mental health issues such as torture and trauma are generally referred to a specialised torture and trauma service.

3. Tuberculosis

Most of the time the migrants undergo a TB test which is known as LTBI for people aged 35 or less. LTBI can be tested by two methods: IGRA and TST. Both are accurate types, but the means of identification are different. However, children aged 5 years or less are recommended TST over IGRA. As a First line preventive measure for the treatment of LTBI/Tb, an antibiotic named Isoniazid for a duration of 6-9 months is recomended. This antibiotic has been effective for migrants/refugees.(Australian Refugee Health » Primary Care for People of Refugee Backgrounds, n.d.)

(n.d.).
