O - Pt was able to walk to closet to retrieve clothes with min assist.  Pt was able to donn UE and LE dressings with CGA. 

A - Pt demonstrated balance impairments during functional mobility.  Pt showed lack of self monitoring and safety during UE and LE dressing.  Pt demonstrated good balance and trunk stability while sitting on edge of bed.  Balance, functional mobility, and safety education should be the focus of OT services.

P: Pt will complete upper and lower body dressing safely and I in 2 weeks.
Pt will be able to stand I and safely for two minutes with supervision in 2 visits.
