I have a child with Eczema and parents brought over the counter cream with doctor note to apply the cream that parent provide without the name of cream. Is this doctor note acceptable to Lisencsee?

I give back the cream and asked the parent to get the doctor note with the name of med, to be able to administer the medication. unfortunately they did not provide it.
