The early days of pressurized jet aviation began with planes being ripped apart in midair due to square windows which created weak points susceptible to cracks. Other examples include simple errors like having landing planes and planes about to take off on the same runway of an aircraft carrier, or building a bridge with I-beams placed together such that wind would catch the bridge and throw it. In summary, many of the things we take for granted, like push to exit doors which create an easier escape in an emergency, originated from others making a mistake and causing a disaster

The book cover for The birth of a New Physics has two arrows facing each other that have an unknown meaning. The arrows can't represent forces or acceleration because of their sizes. A pocket ruler given out by the Pennsylvania Department of Environmental Protection was missing centimeter 14.

People discovered many medically relevant chemicals through mistakes. The discovery of the powers of laughing gas were made at a party. A man who was high on the gas sliced his leg and felt no pain. A doctor who realized this tried an experiment on himself where he drug himself and allowed his colleague to remove his rotten tooth without pain. Along with that, the discovery of penicillin was made by a dirty scientist who kept an unclean work environment, allowing the fungus, Penicillin to grow.

A simple flaw that resulted in nearly 60 deaths derived from early Jet Plane pressurizing. The vessels began to burst in the air due to a flaw in the square windows because the corners could not hold pressure and would crack. Also flaws include hotel designs for a two leveled bridge walkway. It consisted of a long rod connecting both to equal the weight but lazy builders used two rods thinking it would act the same but the bottom actually would have to hold double the weight.

For years, graphic designers have consistently used gears that, in reality, wouldn't turn to represent pieces working together. For example, 3 gears whose teeth all connect representing the transportation in a city. The gears wouldn't turn because one gear would need to turn in two different directions at the same time. Perhaps the most famous example is a British 2 pound coin that has been issued at least three times since 1997, which includes 19 gears linked together in a circle. If they had added one gear it would turn, but the gears have persisted through all three designs.

Writers make a few common, general mistakes that make their writing dull and unexciting. One is lack of editing. Fast word vomit does not come out refined. There can also be too much detail, but a writer mustn't lack vibrant imagery. In writing, we must make dialogue realistic, but at other times we have to vary the way we convey simple actions. For example, instead of saying that a girl, "sighed with relief" a writer needs to find more interesting ways of showing her relief subtly.

Many people unknowingly made some of the biggest scientific discoveries of their time. Many knew of the different "discoveries" before they were discovered for their current use. The average person just never thought to apply them to science, technology, or medicine. By making mistakes, groundbreaking findings were discovered. For example, an 18 year old discovered mauve dye by simply mixing chemicals and reviewing the results to see that the water turned to a purple color, and then that it could also turn his shirt color.

As for the world of technology, many mistakes of numerous variations have occurred. Most mistakes are simply the misapplication or ignorance of scientific principles, one being the cover of an album from Pink Floyd. On the album cover, purple, magenta, and pink are all pictured in the rainbow traversing through the triangular prism. However, pink, magenta, and pink are not spectral colors, meaning it's impossible for them to be in the rainbow of light pictured on the album cover. Yikes.

In some cases, people are not confident in their work which allows it to become undiscovered, as was the case of Alexander Fleming. Although his work area and desk were unclean and disorganized, it allowed for him to accidentally discover a rare strain of fungus, penicillium, that he later tested to create penicillin. But Fleming did not think it would be productive in treating disease so he downplayed the significance of it's affects to a group of scientists. Because of the mistake regarding the fungus, he made one of the most impactful discoveries of the medical community.
