Its ability to vary surface proteins (pili and Opa proteins) and acquire antibiotic-resistant genes has fueled rising treatment challenges for this sexually transmitted infection.

Gonorrhea often causes no symptoms, but when they appear, they usually involve the genital tract. In men, it typically presents with a burning sensation during urination and a pus-like discharge (white, yellow, or green) from the penis. In women, it can cause increased vaginal discharge, pain or burning on urination, bleeding between periods, and lower abdominal or pelvic pain. Rectal infections may lead to anal itching, discharge, bleeding on wiping, or pain during bowel movements. Throat infections are often asymptomatic but can cause a sore throat and swollen lymph nodes. Rarely, the eyes may become infected, resulting in redness, pain, and purulent discharge.

Untreated gonorrhea in women can ascend to the upper reproductive tract causing pelvic inflammatory disease (PID), which leads to chronic pelvic pain, fallopian tube scarring, infertility, and increased risk of ectopic pregnancy. In men, persistent infection may spread to the epididymis or prostate, resulting in epididymitis or prostatitis with testicular pain, swelling, and scarring that can impair fertility. Rarely, the bacterium can enter the bloodstream, causing disseminated gonococcal infection with septic arthritis, skin lesions, and endocarditis, which can be life-threatening. Additionally, gonorrhea-induced mucosal inflammation significantly increases the risk of both acquiring and transmitting HIV.

Neonates acquire gonorrhea perinatally when passing through an infected birth canal. The most serious outcome is ophthalmia neonatorum, a purulent conjunctivitis that can lead to corneal perforation and blindness if not treated promptly. Untreated infections may disseminate, causing sepsis, arthritis, meningitis, or abscesses, which can be life-threatening. Routine erythromycin eye ointment at birth and prompt parenteral antibiotics like ceftriaxone have greatly reduced complications, but maternal screening and treatment remain essential.

Importance of Regular STI Testing

STIs often have no symptoms, so regular testing is crucial to detect hidden infections and prevent unnoticed transmission. Early detection through routine screening enables timely treatment, reducing the risk of serious complications such as pelvic inflammatory disease, infertility, and increased HIV susceptibility. Regular STI testing also protects sexual partners and the broader community by interrupting transmission chains and facilitating partner notification and treatment. Incorporating STI screening into routine healthcare promotes informed decision-making, peace of mind, and responsible sexual behavior.

Gonorrhea, sometimes called “the clap,” is the second most commonly reported notifiable disease in the U.S. and infects roughly 78 million people worldwide each year. It disproportionately affects young adults aged 15–24, with half of U.S. cases occurring in this group and up to 50 percent of infections going undiagnosed due to asymptomatic presentations, especially in women. Transmission occurs through vaginal, anal, or oral sex—even without ejaculation—since direct contact with infected mucous membranes spreads the bacterium, meaning condoms may not fully prevent transmission if sores lie outside condom coverage. The emergence of multidrug-resistant “super gonorrhea” strains has led the CDC to classify gonorrhea as an urgent public health threat, highlighting the critical need for novel antibiotics and vaccine development to control its spread.
