However communication could have some barrier such as disability, visual or hearing impairment, or even different accent, prejudice, medical jargon,  
etc.
Message could be misinterpreted due to changes in medical condition such as dementia, stroke, mental health etc or due to  our body language or approach.
Therefore it is important to get to know the patients medical history and background., build the trust and empathising.
