  The accident happened on Friday 3 February 2020 at 2 pm.  At ABC Shipping Co.  Located on 13th Avenue.                                     
 cause.                                         
                     Sarah slipped and fell while carrying   a heavy (70 lb) stock.  The root cause is   believed  to be the unavailability of a hand truck or a  pallet truck.  Instead of waiting, Sarah tried to shoulder the burden herself. When Sarah fell, I rushed to her aid.  After a suspected her broken leg and a sprained wrist, i decided to call an ambulance, 
      Sarah said she felt slightly dizzy when she lost her balance and "stumbled on her feet." I stayed with her until the paramedics come and took her to hospital .                                     
   and a fracture leg was confirmed. sara will be out of work for a number of weeks.

The supervisor is currently working with the safety officer (Chris Darnell) to assess the condition of the floor, the number and condition of hand trucks and pallet jacks on the floor, as well as the condition of company-provided boots over two years old.”                     solution:              
                                           
   so that the similar accident can be avoided in the future .  in my opinion a solution to avoid a similar accident would be to use a wet floor sign if the floor is wet. Another solution provid  not to use an old boots who has more than two years . last but not least, saving  a more  hand trucks and pallet jacks on the floor .

The accident happened on Tuesday 14th of June 2020 , around 10:30 am , at Morrison supermarket in small Heath Birmingham .
I was working as usual on the till .I heard a shout from the second floor . As soon as I heard the shout , I hurried to the second floor and found John slipped off the ladder while he was looking for olive oil .

John slipped because the ladder is not stable probably ,also there was water in the floor . John told me he was in a lots of pain and he could not move he’s right wrist . I told him to stay were he was and I called the ambulance from my mobile phone ,so I stayed with him until the ambulance arrived .

John was taken to a near by hospital for check up ,after two hours he called me and told me that a fracture to his right wrist was confirmed .

The solutions ,so that a similar accident can be a voided in the future . In my opinion a solution would be to use a wet floor sign if the floor is wet . Last but not least , designate one or two people who can use the ladder under provide them with  appropriate safety training .  

In addition, make sure the ladder is stable and locked the both sides ,also do not lean the ladder  against a moving objects. However , never standing above of the highest step .

The accident happened on 10th of January 2021 around 9:am at Lidl super market,Moseley Road.

I went shopping I was in the vegetable section and everything was going well.suddenly,I heard a loud shout in the dairy lane. As soon as I heard the scream speeding up the aisle, I was shocked to see one of my kidneys, called Amal, lying on the ground. I saw Amal couldn’t move her legs and couldn’t get up.

Even though,she couldn’t speak from the pain,she told me that she slid on the floor and the ground was wet while she doing her job. When she slipped, a few objects fell on her legs as a result of damage to her right leg.

At the same time, I called an ambulance from my mobile phone and asked a customer to help her when the ambulance arrive. I started with her until the paramedic came to help her and took her to the hospital. I gave them my phone number and my address if there is any emergency. After three hours she called me and told she is alright but need to take rest.

Solution,
