Can there be a tab for Mental Health for Molly to add a event that she had communication with date but no specific notations of what was talked about? Just to show that there was an attempt at least to help the case manager is aware. And to have case manger add note that referral was sent for Molly to check in with the family.

When we receive an online application is there a way to see who it all has been sent to? That we know the person who should get it is receiving the info.

Would it possible to change the verbiage on this form so it can be used for longer than 1 year after signed? We are supposed to fill them out for every child at enrollment for their perspective school district to be used for IEPs and/or 4k transition but it expires before it's needed for the 4k transition so we have to have them completed twice if their enrollment was completed in the early spring like the majority are.
