Caerphilly SPACE Wellbeing is not the correct pathway to request assessments for a diagnosis of Autism/ADHD, these assessments are completed by the NESH team.

Families cannot self refer into this service. If you would like to request a Neurodevelopmental assessment, please speak with your child's school in the first instance. If your child is home schooled, any professional supporting your child or your GP can make this referral on your behalf.

The JAFF must be completed to refer into our Families First Projects.

• The JAFF is centered around the families needs and focuses on capturing 'What Matters' to the family. 

• The JAFF is not just a referral form but a rolling document that intends to 'follow' the family around while they access different support services.

• The JAFF can be updated by different professionals, therefore helps to create one document including all important history of a family.
