What are the pros/cons of providing free consultations (as this could lead to use of direct access)? Free consultations allow a patient to see a PT first and begin their plan of care if they are appropriate for PT. The PT can determine if they need to see a physician for a referral or (if insurance allows) if they can utilize direct access. The issue is that this takes up time and does not make money for the clinic. Also, this would only be applicable in an outpatient setting.

The subjective interview, of particular importance in the OP setting, is critical to obtaining the correct information from patients in order to make correct hypotheses and choose proper tests and measures. We know that asking open ended questions is key, but how can we further improve our delivery in order to get an unbiased report, without leading the patient. Our language and choice of words can potentially lead the patient to "give us what we want", rather than giving their actual accurate story. How can students and clinicians improve delivery of questions in a proper flow to improve the subjective report we are given in order to make the best clinical decisions moving forward?
- Logan H

When should PT be favored over surgery? When is surgery the right route to go? Which treatment shows the best outcomes for my condition? I would like to examine common injuries/MSK/neuro diagnoses and prognoses to differentiate, describe, and demonstrate the best evidence-based treatments provided for each separate condition. I believe this would be beneficial for patients with general pain, acute injuries, and patients considering surgery. This information is important as it allows patients to learn and understand the risks and benefits between each treatment and gives the patient the knowledge and power to make a well-informed decision for their personal healthcare needs.

When many people injure themselves, it's very common to initially think that surgery is the main solution. However, as we know, therapy has proven to be very beneficial, if not more, than surgery in certain cases. A lot of individuals believe that injuries that are severe enough require surgery, or that surgery is a fix-all. Educating all individuals that Physical Therapy is a great potential option instead of surgery would be optimal in appropriately directing people to making a better decision for their health that could also be cost effective.

How do we make sure that we are getting the most out of our treatments with patients? At the end of the day, our primary goal is to improve the deficits that we find in our patients and address them accordingly. To do so, we must challenge our patients during the treatments to ensure optimal outcomes. The difficult part for us as PT’s is that there is margin of error of challenging them too much that they are not able to recover well post-treatment, or it is not challenging enough which slows their progress. An undervalued tool we have available that many Strength & Conditioning coaches utilize regularly is the RIR/RPE scale. The RIR/RPE Scale is a subjective rating of exertion that often overlooked as a tool of measurement. The scale can be utilized in many different settings, it can be an effective way to track progress for the PT and the patient, and it can be used as a determinant for when progression and regression is needed for an exercise. Proper education on utilizing this scale will not only optimize a patients treatment time but also improve progress over time with the patient without worry of overworking or underworking the patient.

I have noticed that there always seems to be a disconnect between personal trainers or independent gyms and Physical Therapists. I feel like there should be more of a relationship between the two because of how similar our positions are. So when someone leaves PT and goes to a gym they are unlikely to progress their therapy from where it left off as safely and effectively as possible. That's because the average trainer does not have a lot of corrective exercise knowledge or an understanding of the phases of healing and progression. I believe it would be helpful for trainers and their clients if we could provide a lecture/lectures on the basics of corrective exercise and rehab. These can be body region specific or as a gross overview about how to approach structuring a workout routine to optimize wellness, functional movement, strength, injury prevention, and rehab.

1. Exercise controls weight
2. Exercise combats health conditions and diseases
3. Exercise improves mood
4. Exercise boosts energy
5. Exercise promotes better sleep
6. Exercise can be fun … and social!
For most healthy adults, the U.S. Department of Health and Human Services recommends these exercise guidelines:
-Aerobic activity. Get at least 150 minutes of moderate aerobic activity or 75 minutes of vigorous aerobic activity a week, or a combination of moderate and vigorous activity. The guidelines suggest that you spread out this exercise during the course of a week. To provide even greater health benefit and to assist with weight loss or maintaining weight loss, at least 300 minutes a week is recommended. But even small amounts of physical activity are helpful. Being active for short periods of time throughout the day can add up to provide health benefit.
-Strength training. Do strength training exercises for all major muscle groups at least two times a week. Aim to do a single set of each exercise using a weight or resistance level heavy enough to tire your muscles after about 12 to 15 repetitions.

Moderate aerobic exercise includes activities such as brisk walking, biking, swimming and mowing the lawn. Vigorous aerobic exercise includes activities such as running, heavy yardwork and aerobic dancing. Strength training can include use of weight machines, your own body weight, heavy bags, resistance tubing or resistance paddles in the water, or activities such as rock climbing.
(Holly H.)
