I use a non-anonymous form as a get-to-know-you at the beginning of the semester. But I might need to think about making them anonymous.

I read every feedback survey and follow through on every valid complaint/issue.

I have a hard time when I ask what should you like to see changed, and they say "slow down", but I'm already slower than all the other times I've taught it, and it is impossible (in my mind) to slow it down anymore.
