+epiglottitis 호흡곤란이 오지는 않았지만 감별은 필요합니다.

➝ 일측성(Trismus, Odynophagia 등) (1.2.)

우측이 심하고 이통이 동반됨 -> abscess

1) peritonsilar abscess

2) parapharyngeal abscess

3) epiglottitis

4) retropharyngeal abscess

5) lemierre syndrome

peritonsillar abscess

• 처치는 배농(drainage) 후 pain control

• dyspnea, symptom aggravation, neck swelling, stiffness의 경우 꼭 입원시켜서 관찰.

• 24~36시간 뒤에 follow up해서 재배농해야 합니다.

• antibiotics도 사용 가능

2. 처치

1) 배액 (절개, 흡인) + 항생제 사용

2) 24~36시간 후 재배농

3) 증상악화(호흡곤란, 목 경직, 목부종) 시 입원 (∵ 하인두 및 기도 침범 위험)

4) 보존적 치료: 통증 조절

• uvula deviation이 있으므로

처치

1. 절개배농

2. 항생제 (암피실린 + 세팔로스포린 or 클린다마이신)

3. 외래 추적관찰 24-36시간 간격으로 한번더 천자. 이때 또 나오면 배농

처치는 aspiration drainage를 우선적으로 시행한다. 다시 농이 찰 가능성이 높으므로 1-2일 내에 재방문하여 re-drainage한다.

이비인후과 협진을 통해 절개배농을 시행 후

FU으로 재배농(24시간 36시간 간격)

호흡곤란으로 진행하면 deep neck space의 감염을 의심하고 입원 후 기관삽관을 한다

• uvula deviation이 보임

• 현재 stridor는 들리지 않음

• anterior pillar bulging이 보임

2.처치

-우선 시행 배농 -> 24-26시간 후 f/u 해서 다시 배농

-항생제 ( ampicillin + 베타락탐, 세팔로스포린)

-airway control 필요시

-deep neck space infection 조짐 보이면 입원 치료( dyspnea, neck swelling, stiffness)

→ 근거: uvula가 한쪽으로 치우쳐 있고, 입벌리기 어려움.

처치

1) ENT 협진

2) 배농: 절개 or 흡인

• 24~36시간 후 재배농 위해 내원 안내

3) 항생제: ampicillin + β-lactamase inhibitor

• 혐기성 세균감염 가능성 높으므로 clindamycin도 사용

4) supportive care: 수액공급, 통증조절

치료: 우선 drainage(incision or aspiration)를 시행하고, 이후에 항생제(ampicillin+b-lactamase inhibitor, cephalosporin, clindamycin), 보존적 치료(pain management)를 할 수 있습니다.

감별해야할 질환:

• acute epiglottitis(제일 유력)

• peritonsilar abscess

• parapharyngeal abscess

• Lemierre syndrome

시행해야할 검사:

• 후두경

• lateral c-spine X-ray

검사: Lateral C spine X-ray, Laryngoscopic evaluation

2) 검사: Lateral neck simple X-ray, Laryngoscope, CT

검사: lateral neck xray, 후두경(acute epiglottitis 감별), neck ct scan(parapharyngeal abscess 감별), lab

대칭이므로 parapharyngeal/peritonsillar abscess 등 배제 가능

촉진되는 종창이 없고 대칭적이므로 Lemierre SD 배제 가능

1) 감별진단

• acute epiglottitis: 연하통, 호흡곤란

• retropharyngeal abscess: 구강검사로 진단 어려우므로 가능성 있음

2) 시행해야 할 검사

• Lateral neck simple X-ray

• X-ray로 발견 어려우면 CT 시행

가장 의심되는 acute epiglottitis를 진단하기 위해 lateral neck X-ray로 thumb print sign을 확인할 수 있다. 또는 후두경으로 부푼 후두개를 보고 진단할 수 있다.

진단; acute epiglottitis

검사; laterl cervical X ray

가능하면 후두경검사

+retropharyngeal abscess도 가능하지만 편측으로

+parapharyngeal abscess도 호흡곤란을 유발하지만 편측에 주로 발생한다

• 1일전부터 시작된 통증, 밖에서 촉지되는 것/안에서 보이는 것 딱히 없음 (abscess ↓)

필요한 검사

1. Lateral C-spine X ray

2. Laryngoscope

#1. Acute epiglottitis : 1일 전부터 급작스럽게 생겼으므로 가장 의심됨.

peritonsillar abscess : 구강검사에서 안 보였으므로 가능성이 떨어짐

Parapharyngeal abscess : 경부 촉진에서 특이 소견 없으므로 가능성이 떨어짐

Lemierre SD : 경부 촉진에서 특이 소견 없으므로 가능성이 떨어짐

검사

1) C-spine lateral X-ray

2) 후두경 : ENT에 의뢰에서 시행

1. Acute epiglottis : 급성 증상, 호흡곤란이 있으므로 가장 가능성 있는 진단

   -> 검사: lateral C-spine X-ray, laryngoscopy

2. Deep neck space infection : 호흡곤란이 있고 아직 증상발현까지 시간이 걸릴 수 있으므로

   (parapharyngeal cellulitis, retropharyngeal abscess, submental cellulitis, lemierre SD)

-> 검사: CT scan, X-ray, laryngoscopy

치료: Airway control(intensive monitor or ICU care, elective intubation if needed) + 항생제(2nd or 3rd cephalosporin)

2) 치료

-기도 확보 (산소포화도 확인 -> 중환자실 입원 -> 필요시 기관삽관 -> 어려우면 기관절개술)

-항생제 (2세대, 3세대 cephalosporin)

-스테로이드 (∵ 부종 감소)

① 입원 후 모니터링 시행, 기도확보

② 항생제 처치, 부종 감소 목적 스테로이드 투여

③ 지속적인 Laryngoscope F/U

치료; intensitive monitor 산소모니터를 단다

스테로이드+항생제를 처방한다

호흡곤란이 심해지면 intubation+ENT협진

처치:

• airway control 해준 뒤, 2세대/3세대 cephalosporin으로 항생제 치료

• 부종 완화를 위한 steroid

• 필요시 intubation, 안되면 tracheostomy

처치

1) 산소포화도 모니터링

2) ENT 협진, 필요시 ICU care

3) 항생제, steroid로 기도확보

• 항생제: 2세대 or 3세대 cephalosporin

4) 약물에도 호흡곤란 호전 안되면 기도삽관

5) 그래도 안되면 기관절개

처치 : 산소 모니터를 우선적으로 달고, 호흡곤란이 심하면 기관삽관을 진행한다.

처치

1. airway control

   • 중환자실에 입원해서 close monitoring

   • 기관삽관 : 호흡곤란 심해지면

   • 기관절개술 : 기관삽관으로 호흡곤란이 좋아지지 않으면

2. 항생제 (2nd, 3rd 세대 cephalosporin)

• Airway control (intensive monitor / ICU care, elective intubaton, 심각하면 tracheostomy 기관삽관)

• 스테로이드(후두개 염증으로 인한 부종 완화 목적)

• 항생제 (2,3세대 cephalosporin)
