What were the main reasons for the infant mortality being so high?
Miscarriage or still birth death are accounted for even though survival under 22 week or under 500g are not expected live. The usa also has a relatively high post neonatal mortality rate.

There are lots of resources and healthcare good healthcare services.

However not everything in the USA is MEDC, there are some places in which people can access or afford a good health care, or the place where they live in is not a good environment.

I am surprised just because of the fact that this is something you wouldn't expect from a country that is rich enough and has the enough resources to support and maintain babies. However I am not surprised, as mortality differences in the U.S are largely due to socioeconomic differences. As babies born to disadvantaged mothers in the US are more likely to die.
