]]> 2023-10-11 12:58:14 UTC https://padlet.com/22000249/pkwnquxahegcj59u/wish/2741765185 22000249 https://padlet.com/22000249/pkwnquxahegcj59u/wish/2741784296 implementing care values is also known as person centered values,these are principles that help put the interests of the service user at the center of what practitioners do. it is a way to approach when providing care and it is about valuing and respecting the service users decisions whilst supporting them. practitioners have to follow this to make sure that patients/s.u's feel comfortable when receiving support and that they can trust, rely and feel as though they can depend on any advice that may be given to them. there are 7  main values that are associated too this such as -respect, maintaining confidentiality, promoting independence, dignity, empathy, safeguarding and a duty of care as well as anti-discriminatory practice. practitioners ensure that all of this is included in their practice whilst helping to make sure the users needs are met and help them plan for future references. Practitioners have to accept that everyone's decisions are different and should be respected as its their body meaning their choice. However can still discuss any concerns about their choice if needed and therefore support them in a way that they agree on as their health and support is still the practitioners number 1 priority.

]]> 2023-10-11 13:10:05 UTC https://padlet.com/22000249/pkwnquxahegcj59u/wish/2741784296 22000249 https://padlet.com/22000249/pkwnquxahegcj59u/wish/2741786263 a care plan is a document created by a practiotonor that can be shared with whoever the service user wants it to and is a record of any details about the patients medical history, personal details, any outcomes that may be possible and information about the care they receive. it is the practitioners responsibility to follow and create a care plan for a patient that may need it, this is so that any information they need to find is written down to make it easier to find crucial information, helps to have better communication and understanding of he user. it helps avoid/ prevent any issues for the practitioners are responsible for having  conversations set up as service users have the right to decide for themselves and understand better about what is happening.  practitioners are required to follow the steps for the care plan which is- prepare, discuss, document and review. this all ensures that practitioners work with the users rather than working for them.
