A discharge summary audit was conducted on pt1. The discharge summary accurately reflects his stay throughout the hospital stating: the reason for admission; any events that occurred; the treatment that was provided; relevant tests including ECG and telemetry; and the reason for safe discharge. The summary stated he is for follow-up in cardiac clinic in 6 months and will be contacts; and is to see his GP within 1 month or with other symptoms etc. The summary stated he is for cardiac rehabilitation and will be contacted in regards to appt. No family or carers were present during his hospital admission and he appears capable of taking care of his own health. A medication profile was given to the patient on discharge due to previous poor medication compliance. He previously had a supply of prior medications. He was given scripts for new medication however this was not stated in the summary. The supply of medication will last further than GP follow-up. The discharge summary was completed prior to discharge of patient and will be sent to GP within 48 hours. Overall, this discharge summary adequately fulfils the AMA requirements for a good transfer of care. This discharge summary accurately detailed the important components of Pt1 stay; his state on discharge; medications; and plans for follow-up. This means that both the patient and primary health care provider will accurately be able to understand what happened and the plan for the future. The summary didn’t include unnecessary information meaning ease of understanding and the event. This discharge summary should be effective for patient and GP understanding; continued health provision; and ultimately good patient outcomes. When it comes time for myself to complete a discharge summary this and the AMA guideline is a good example/template of how I should complete mine. I will also ensure to complete it in a timely manner to help aid in medication adherence; pt health understand; and appropriate transfer and continuation of care

The summary not only included the relevant management in relation to cardiology but also describe the respiratory team input as this is relevant to the overall care. The summary stated he is for follow-up in cardiac clinic in 6 months and will be contacted; and is to see the respiratory team in 2 weeks per their plan. He is not for any time specific GP follow-up. His wife was present during his stay and was included in pt management and discharge planning. No medication changes occurred and he did not need any new scripts or medication supply. The discharge summary was completed prior to discharge of patient and will be sent to GP within 48 hours. Overall, this discharge summary adequately fulfils the AMA requirements for a good transfer of care. This discharge summary accurately detailed the important components of Pt2 stay; his state on discharge and new diagnoses; prior/current medications; and plans for follow-up. This means that both the patient and primary health care provider will accurately be able to understand what happened and the plan for the future. The summary didn’t include unnecessary information meaning ease of understanding and the event. The summary however lacked note that his cardiac/resp admission was due to an incidental finding on anaesthetic review prior to a TURP. This could be useful for further health care management making it clear that he was asymptomatic and did not undergo a TURP. This is something I would include in my future summaries for completeness if applicable. This discharge summary should be effective for patient and GP understanding of new diagnoses and management/plan; continued health provision; and ultimately good patient outcomes. When it comes time for myself to complete a discharge summary this and the AMA guideline is a good example/template of how I should complete mine. I will also ensure to complete it in a timely manner to help aid in pt and family health understanding; pt understanding on follow-up; and appropriate transfer and continuation of care.

The discharge summary accurately reflects her stay throughout the hospital stating: the reason for admission; nil adverse events occurred; the treatment that was provided; relevant tests including ECG and telemetry; and the reason for safe discharge. The summary stated she is for follow-up in cardiac clinic in 2 months and will be contacted; and is to see her GP within 2 weeks regarding her admission and if she has any sx similar to this presentation. The summary did not state that she received chest physio during this stay and that is was helpful. Noting this would be beneficial as it may be used in outpatient care and also if she is to be admitted again with similar. This is something i will ensure to include in my discharge summaries if allied health is provided during their stay. This patient is capable of caring for herself independently and does not have any family support. this was not stated in her discharge summary although it is already known by her usual GP. This is something I would include in my discharge summaries incase this letter is seen by a different GP or health professional as knowledge of lack of family support is important when deciding treatments/care in elderly. The patient already had a medication profile although this wasn't stated in the summary. There were no medication changes on discharge and the patent has sufficient medication and script supply although note of supply was not stated in the summary. The supply of medication will last further than GP follow-up. The discharge summary was completed prior to discharge of patient and will be sent to GP within 48 hours. Overall, this discharge summary mostly fulfils the AMA requirements for a good transfer of care. This discharge summary accurately detailed the important components of Pt3 stay; his state on discharge; medications; and plans for follow-up. This means that both the patient and primary health care provider will accurately be able to understand what happened and the plan for the future. This discharge summary should be effective for patient and GP understanding; continued health provision; and ultimately good patient outcomes. This and the AMA guidelines for discharge summary will be useful when writing mine in the future; with addition of the above noted changes.
