         <title>Hamzah Khan case review</title>

         <description><![CDATA[<div>Hamzah Khan was born on 17 June 2005 and was found deceased on 21 September 2011. His mother, Amanda Hutton, indicated that Hamzah had died on 15 December 2009 and she has subsequently been convicted of his manslaughter. </div><div><br></div><div>Throughout Hamzah’s life, West Yorkshire Police and other agencies had contact with the family with regards to Domestic Violence. In addition, the father of Hamzah Khan raised issues of neglect, specifically referring to Hamzah, to the police during an interview on 9 December 2008. D4 3.</div><div><br></div><div> A Serious Case Review (SCR) was undertaken by the Bradford Safeguarding Children Board and their report was published on 13 November 2013. The report received significant media and ministerial interest, together with scrutiny into the circumstances of Hamzah’s death. There was significant criticism of the report and what were perceived as ‘missed opportunities’ by agencies between Hamzah’s birth in 2005 and his body being discovered in 2011. West Yorkshire Police investigated the circumstances into Hamzah’s death, contributed to the SCR and had involvement with some family members prior to the body of Hamzah being discovered. </div><div><br><br></div><div>Whilst there were no serious concerns raised over West Yorkshire Police’s involvement around Hamzah’s death by the SCR, the subsequent interest and profile of the case had the potential to raise questions regarding the conduct of West Yorkshire Police which could have a significant impact on public confidence.          <description><![CDATA[<div>An account of the developments surrounding the case of 17-month-old boy Baby Peter who died in Haringey, north London, in August 2007 after suffering a series of injuries. December 2006: Connelly is arrested after bruises are spotted on Baby Peter's face and chest by a GP. January 2007: The boy is returned home five weeks after being put in the care of a family friend. February 2007: A whistle-blower, former social worker Nevres Kemal, sends a letter about her concerns over alleged failings in child protection in Haringey to the Department of Health. 12 March 2007: Commission for Social Care Inspection (CSCI) inspectors meet Haringey officials to discuss concerns raised by Ms Kemal in the letter sent by her lawyer, which was dated 16 February 2007. April 2007: Baby Peter is admitted to North Middlesex hospital with bruises, two black eyes and swelling on the left side of his head. May 2007: After seeing marks on the boy's face, a social worker sends Baby Peter to the North Middlesex where 12 areas of bruises and scratches are found. Connelly is re-arrested.30 July 2007: Injuries to Baby Peter's face and hands are missed by a social worker after the boy is deliberately smeared with chocolate to hide them. 1 August 2007: The boy is examined at a child development clinic. 2 August 2007: Police tell Connelly she will not be prosecuted after her case is considered by the Crown Prosecution Service. 3 August 2007: Baby Peter is found dead in his cot.11 November 2008: Owen, and Barker, are found guilty of causing the death of Baby Peter. Connelly had pleaded guilty to the same charge.13 November 2008: Children's Minister Ed Balls orders an inquiry into the role of the local authority, the health authority and the police in the case of Baby Peter. by the Care Quality Commission for failing in the care given to Baby Peter. 22 May 2009: Connelly gets an indefinite jail term with a minimum term of five years for her part in her son's death. Barker is jailed for life with a minimum of 10 years for raping the two-year-old and given a 12-year term to run concurrently over his role in Baby Peter's death. Owen gets an indefinite sentence with a minimum term of three years.3 July 2009: Inspectors criticise Haringey Council, saying it has only made limited progress in tackling areas of weakness.11 August 2009: Connelly and Barker are named for the first time after the expiry of a court order.22 October 2010: Two social workers who dealt with Baby Peter, Gillie Christou and Maria Ward, lose their claim for unfair dismissal. They had argued they were sacked unfairly by Haringey Council following his death, but a tribunal found the authority acted reasonably because of failings in the care they provided.26 October 2010: The second serious case review - the official account of the agencies' failings over Peter - is published, after the first one was ruled "inadequate" by Ofsted. 27 May 2011: The Court of Appeal rules in favour of Ms Shoesmith, who claims former Children's Secretary Ed Balls and Haringey Council acted unlawfully by sacking her.24 June 2011: The Department for Education and Haringey Council confirm they will seek an appeal at the Supreme Court against the Court of Appeal's ruling that Sharon Shoesmith was unfairly sacked.2 August 2011: The Supreme Court refuses to grant officials leave to appeal against the decision that Ms Shoesmith was unfairly sacked.5 August 2011: Jason Owen, now 39, is released from prison after serving three years of a six-year sentence.5 25 May 2012: Haringey Council workers Gillie Christou and Maria Ward lose their appeal against an employment tribunal ruling that they were fairly sacked. 29 May 2012: The Chief Executive of Great Ormond Street Hospital Dr Jane Collins announces she is to stand down. 12 March 2013: The Court of Appeal backs an employment tribunal ruling that Baby P's social workers were not sacked unfairly by Haringey Council. 12 April 2013: Jason Owen is returned to prison for breaching conditions of his release. 8 October 2013: The Parole Board says it has directed the release of the mother of Baby Peter, Tracey Connelly.          <description><![CDATA[<div>&nbsp;In 2000, Victoria Climbe was tortured and murdered by her guardians at the age of 8. The person who examined her body said it was the worst case of child abuse they had seen. She died at St Mary’s Hospital in Paddington, her heart, lungs and kidneys had failed. At that time, she had 128 injuries to her body. <br><strong>Types Of Abuse: <br></strong>&nbsp;The types of abuse that Victoria suffered were physical abuse. She was burnt with cigarettes, hit with bike chains, wires and would have her toes smashed with a hammer and was tied up for long periods of time. <br>She also suffered emotional abuse as her arms were tied behind her back and was made to eat her food like a dog. Having her face pushed into the plate. <br>She was also neglected, not being able to have a bed and instead being forced to sleep in a bath with only a black bin bag. <br>It was also alleged that an sexual assault had taken place. <br><br><strong>Missed Opportunities: </strong><br><br></div><div>Victoria makes her first visit on the 14<sup>th</sup> July 1999 to the Central Middlesex Hospital. The doctors accept Kouao’s story that Victoria has inflicted the wounds on herself by scratching at the scabies sores.&nbsp;<br><br></div><div>During this time, doctors alerted child protection as a precaution. Lisa Arthurworry (social worker) and PC Karen Jones were assigned to the case. They had a home visit scheduled for the 4<sup>th</sup> August, but it was cancelled after they heard about the scabies.&nbsp;<br><br></div><div>Ten days later on the 24<sup>th</sup> July, Victoria was taken to North Middlesex hospital’s casualty department with scalding to her head and face. Doctors believed this to be abuse. But the social worker and Police officer assigned accepted Kouao’s story that Victoria inflicted this on herself and a few days later Victoria is allowed to go home with her aunt.&nbsp;<br><br></div><div>Lisa Arthurworry did make a home visit but said that Victoria was happy and comfortable, This was while she being abused.&nbsp;<br><br></div><div>Kouao tells social workers that Carl Manning sexual assaulted Victoria, but withdraws it the next day. PC Jones is asked to investigate but takes no further action after a letter to Kouao is unanswered. The social worker visits the home twice after this but leaves after there is no answer. She believed they had moved away and didn’t follow up or check.&nbsp;<br><br></div><div>After this home visit Lisa found a note from a paediatrician saying that the injuries he treated were in his belief to be because of abuse. But Lisa decided not to follow up, instead sticking to her previous decision.&nbsp;<br><br><br></div><div><br><br></div>]]></description>

         <title>Four year old Daniel Pelka was murdered by his mother and her partner in March 2012, after a period of sustained abuse. Daniel’s mother and her partner were found guilty of his murder, and sentenced to a minimum of 30 years in prison. The case attracted widespread coverage and comment in the media.Łuczak and Krężołek starved the child, beat him, locked him in a room, force-fed him salt, and put his head underwater in the bath. He weighed only one stone nine pounds (10.4 kg) when he died.22nd January 2009A health visitor makes a referral to children&#39;s social care - known as Children, Learning and Young People Directorate (CLYP) - following the most recent domestic abuse incident, recorded in November 2008.A social worker visits the home on 29 January to carry out an assessment and no further action is taken because it is considered Ms Luczak could protect the children.5 July 2010A health visitor goes to the family home to find Daniel has a bruise to the side of his head, with the explanation given that he &quot;fell over&quot;. It is noted that Ms Luczak speaks little English.8 AugustPolice are called to a domestic abuse incident involving knives after an altercation between Ms Luczak and Mr Krezolek. 27 DecemberA neighbour calls the police to a domestic abuse incident, No referral is made to CLYP.6 January 2011Daniel is taken to A&amp;E by Ms Luczak and Mr Krezolek, where a fracture is found on his left arm along with multiple bruising to the arm, left shoulder and lower stomach. Daniel is noted to be interacting well with his mother and Mr Krezolek.7 JanuaryConcerns about the history of domestic abuse incidents at the family home prompt a meeting and it is decided an &quot;in-depth assessment&quot; will be undertaken. CLYP are agreeable to completing a Core Assessment and for the outcomes to be fed back to all professionals.NovemberThe school makes its concerns known to Ms Luczak about Daniel&#39;s continued obsession with food and that he takes food from other children&#39;s lunch boxes. January 2012The deputy head teacher becomes concerned Daniel is not growing and of his obsession with food and organises a meeting with Ms Luczak. Between December 2011 and February 2012Daniel was seen on occasion at school with facial injuries. Because of a lack of appropriate recording within the school, it is unclear what injuries were seen None of these injuries are referred to CLYP or the police.2 MarchThe family&#39;s computer at home is used to seek information on salt poisoning and of a child not responding.Text messages are sent from Ms Luczak to Mr Krezolek are later sent saying &quot;he&#39;ll get over it&quot; and that there was no point in calling an ambulance because it would &quot;cause proper problems&quot;.3 March- ambulance service  He is pronounced dead at 03:50.</title>

         <title>Elsie Scully Hicks and Ayeesha Jane Smith are two different cases of children being neglected but they both have similarities such as the agencies that were involved in both of these cases failed to acknowledge a problem. Elsie was just 18 months when he was murdered by his adopted father.Elsie Scully was under the care of her her adopted father Matthew scully Hicks and his husband. Matthew had a temper problem which resulted in him abusing his children as a way of releasing his anger. He managed to pass off the injuries he inflicted on his daughter as accidental so as not to raise the suspicions of those involved in the close supervision of the family, as part of the adoption process. Craig, Matthew’s husband was unaware of the fact that Elsie and her siblings were being neglected and abused.</title>

         <description><![CDATA[<div>And it was not long before Elsie started suffering injuries which needed hospital attention.Each time she was hurt, Craig Scully-Hicks was at work and his husband had a simple explanation. Or as the prosecution put it, he made up "a story of ordinary domestic accidents to explain something [he] had done".A month after arriving, Elsie's leg was injured. That was blamed on a fall in the kitchen. A doctor found one fracture but missed another. If the second fracture had been spotted doctors would have raised concerns about child protection, the court heard.Two months later she had a bruise on her forehead, which Matthew Scully-Hicks said resulted from her banging her head on a play kitchen.<br><br>This could have been easily prevented if the agencies that worked together in this case worked effectively together and exchanged notes for instance Elsie would suffer injuries to the head and his leg which is the hospital’s responsibility to look into and work alongside social workers to ensure the child’s safety is not in danger.<br>Other professionals that could have stepped in and prevented this are teachers and even neighbours that know the family personally that could have saved Elsie.</div>]]></description>

From Doncaster south yorkshire, four weeks before her death she has been punched, slapped and abused on numerous occasions. She had fractions on her arms, legs, ribs and some time afterwards Howson put his daughter on his knee and hyperextended (extended her limb/ joint beyond its normal limits) her back, her father broke her spine in two places and instantly killed her. Social workers, schools and health visitors all failed to follow basic safeguarding procedures in her care. This case was said to be very similar to the baby peter case as both experienced physical abuse, both died from broken spine and were failed by London’s services that should have protected them. The father was sentenced to death for murder and amy’s mother who admitted allowing the death of a child was believed to be completely dominated by Howson only got a 12 month suspended sentence.          <description><![CDATA[<div>Skyla Giller was allegedly dropped by her foster mother the ten-month-old had emergency brain surgery before doctors gave up hope. Skyler had a lot of swelling to her head and she had a black eye. She was fostered by a woman in her 30’s. The social workers did not inform the birth mother about the information on what Skylar Giller had gone through and instead was told by a consultant that Skylar was “dropped” by her foster mother.&nbsp; Skyler’s birth mother was given false hope by social workers who indicated she would be able to come home at some point.&nbsp;<br><br></div>]]></description>

         <description><![CDATA[<div><br><br><br></div><div><strong>Amy &amp; Owen Philcox</strong></div><div>Security guard Brian Philcox drugged his two children before killing them and himself on Father's Day after leaving explosive devices designed to injure their mother who was divorcing him.</div><div>Philcox used chloroform and a sleeping drug to sedate the children aged seven and three before connecting a vacuum hose to the vehicle's exhaust to kill them all&nbsp;</div><div><br></div><div>Before the devastating news, criminal records show that philcox has been arrested previously because of drug abuse and disturbing the peace because of alcohol. This should have been taken into account because there is a young child living in the house and not once someone went to have a check up on the children, because of the father's behaviour.
