D: How Long does the symptoms occur? Is this the first time ? Does it go away?
C: Can u describe more about the incident? How is the weakness? What you doing when the weakness start? Have you notice yourself tripping?
A:Any sensation you experience when the weakness occur? Any headache?
Do u have any facial drooling noted when it happens? 
Did you have any difficulty in swallowing ? 
Did your facials droop become worse or same? 
Do you feel anything else? Anything you want to add on?
Do you feel giddy? - ( if giddy present; Is the giddy relieved by position? Does   it occur constantly or recurrence? How Long is the giddiness there?
Do you have blurred vision?
R: Does anything relieve the weakness? What makes the weakness better?(rest, exercise) 
T:did you seek for any treatment when you had the first episode of weakness over the left -sided weakness? Did you taken any medication to relieve the weakness?

HEENT - Any problems with vision/hearing?
MSK - Do u have any pain /swelling over limbs ? 
Do you have any fall within 3 months? Do you have any muscle weakness?
GENITOURINARY - How was your appetite?
(If Low appetite) do you have any unintentional weight loss recently ?
Do you have any difficulty during pass urine?  Or motion? 

RESPI- Any cough or runny nose ?

CARDIO- Any chest pain or heart problems?

CT Brain can detect blood immediately
