Harm: Cause abdominal pain, diarrhea, and constipation. Resulting in removal of 49 centimeters section of the small intestine.
The surgical sponge was the  breach of duty that cause direct harm.

Harm: After trying to rectify mistake of first heart and lung transplant surgery caused the patient to slip into a coma. After severe brain damage and other complications the patient died. 

Proximate cause: Doctor being negligent to confirm compatibility of blood type  between patients.

4. the patient lost both legs due to the error of the surgeon because by the time he realized their mistake it was too late to reverse the damage caused and the leg had to be removed.

Breach of Duty- CT scan was taken, results were ignored patient had blood in the brain, it was diagnosed with a sinus infection and a migraine, was discharge from ER and sent home. 

Harm occurred - seconded seizure had occurred went to a different hospital in Fort Worth, during the transfer she had a third seizure. 

The breach of duty- the patient was brain dead 30 minutes during the trip. The patient was on life support was taking off the next day. 

Husband is filing a lawsuit due to the reasoning of malpractice.

Harm:Alexandra’s spine  was destroyed along with  her nerves, leaving her partially paralyzed.
The Breach of Duty:Alexandra died on Valentine’s Day, 1994, at age eight-months.

4.)    The breach of duty- the wrong route of medication caused pain and death of patient, due to ignorance. 

According to Reports a $250,000 verdict was returned for pre-death suffering only.

Breech- Dr. should have checked equipment and verify that it was up to date.

Harm- 81 minuets passed and baby was still alive. Mother was rushed for an emergency c-section. Baby had no oxygen to his brain.

Proximate Cause- .This caused severe spastic quadriplegic cerebral palsy.

Duty:  Dr. "N" was to perform an Angiogram on Pt. "S" and not an Angioplasty.
Breach: Doctor and the nursing staff must have double checked for correct procedure on correct patient. Also asked the patient if this process was explained to him prior to the procedure. The medical staff should have asked for a family member of pt. "S" to witness the signing of consent.
Harm:  Pt. was given Heparin (blood thinner) which caused him to hemorrhage and go into cardiac arrest.
Proximal Cause:  Pt. was not asked how he felt after been given heparin, soon after hemorrhaging, eventually killing him.
