Dr. diagnosed intrapartum death.<br><br>Breech- Dr. should have checked equipment and verify that it was up to date.<br><br>Harm- 81 minuets passed and baby was still alive. Mother was rushed for an emergency c-section. "S" and not an  Angioplasty.<br><strong>Breach:</strong> Doctor and the nursing staff must have double checked for correct procedure on correct patient. Also asked the patient if this process was explained to him prior to the procedure. The medical staff should have asked for a family member of pt. "S" to witness the signing of consent.<br><strong>Harm</strong>:&nbsp; Pt. was given Heparin (blood thinner) which caused him to hemorrhage and go into cardiac arrest.<br><strong>Proximal Cause:</strong>&nbsp; Pt. was not asked how he felt after been given heparin, soon after hemorrhaging, eventually killing him.<br>This case is pending Verdict.</div>]]></description>
