When submitting a Pre-ETS referral, please ensure the following are included:

• Signed (Wet Signature) Pre-ETS Referral Form.

• Student’s Social Security Number

• Most recent IEP

• Most recent psychological report

• Identify if the student is connected to any other agency such as CSB, receiving SSI/SSDI, POA, Guardianship or has waivers granted. Any information that will support the students access to successful services and should be uploaded in the referral.

• Pre-ETS needs- Identify individualized needs

Submit All Documentation to the DARS Submission Form

Once the referral is submitted, the Transition Specialist will send out an email to the student and family ntroducing the DARS counselor and share how to obtain DARS Services. The Case Manager, DARS Counselor, and Program Advisor will be CC'd on the correspondence.

To request specific activities, please use this form request and send an email to let shildebrandt know.

In VA IEP:

1. Create the Invite Agency Personnel form and request the parent/adult student’s signature prior to the IEP meeting.

2. Once signed consent is received, email agency (EX: ayesha.cole@dars.virginia.gov for DARS) to participate in the student’s IEP meeting.

3. Document the consent and agency invitation in VA IEP and reflect within the PLAAFP.

At this time, [STUDENT] does not have a current application or referral on file with the Department for Aging and Rehabilitative Services (DARS). Beginning at age 14, students with disabilities can connect with Vocational Rehab for Pre-Employment Transition Services (Pre-ETS). This includes: 

*Job-exploration Counseling

*Work Readiness Training

*Work-based Learning Experiences

*Postsecondary Educational Counseling

*Self-advocacy Training and Peer Mentoring

[PARENT] was provided with information about DARS, including an referral for services, which he/she may complete at his/her discretion if interested. Should [PARENT] wish to pursue services or learn more, he/she may contact [STUDENT’S] IEP case manager to arrange a meeting with a DARS representative.

In addition, [Parent] was asked to give consent to invite an outside agency to the IEP meeting if it is thought the agency might offer any post-grad support. [Parent] did not give consent to invite an outside agency representative to [STUDENT'S] IEP meeting.

Functional Academic: Math, English/Language, Arts, Listening and Speaking Skills

Domestic Domain: Grooming/Hygiene, Personal Safety, Life Skills

Community Domain: Social Behavior, Community Resources, Recreation and Leisure, Communication Skills

Vocational/Employment Domain: Career Exploration, Work-Related Training, Future Living, and Employment.
