Hand-off is largely dependent on interpersonal communication as well as the knowledge and experience level of the caregiver. According to the Joint Commission and the Agency for Healthcare Research, ineffective hand-off communication has contributed to prolonged lengths of stay, avoidable re-admissions, delayed or inappropriate treatment, increased costs, inefficiencies related to rework, and care omissions. Approximately 70% of serious medical errors are the result of ineffective handoff. 

Nebraska Medicine conducted a study to analyze the effectiveness of a standardized handoff tool. Utilizing a patient-centered bedside report tool called ISHAPED, the study found that this tool reduced nursing overtime, the number of adverse events related to handoff, and patient/family engagement. Therefore, implementing a standardized hand-off tool can improve continuity of care, reduce adverse events, and improve satisfaction among staff and patients.
