PHYSICAL- due in FEBRUARY by first clinic date

You will need to show proof of a current physical. Physicals are good for two years from the date of the examination. The packet says March dates on the application and that is incorrect. It is February dates, by your first practice/clinic.

REPORT CARD

You will need to show proof of a current report card (grading period ending on December 19, 2025) to your campus coach for tryout eligibility.
