This link leads to a very well developed web page.  The forms have clear procedures on them. One of the procedures is to schedule a meeting with me, then go over your plans.  

Field trip requests should be initiated FOUR WEEKS prior to the trip.  Several staff members have already gone through this process for trips happening early in this school year.  Grade level teams are strongly encouraged to do the process together and only need one form.
