Master Kaddy switched off the machine and started to conduct routine checks on the conveyor belt. She saw that one of the connections was not good. She started to change it when Madena switched the machine on. The conveyer belt moved and Kaddy’s left hand was caught in it. Her hand was seriously injured. Madena called for the ambulance. All the fingers on Kaddy’s left hand were fractured. They were placed in a cast and she was advised to remain at home for at least 2 weeks.

Causes
After an initial investigation, I discovered that there was a mistake in maintenance procedure. The procedure requires that the maintenance worker displays a “Work in Progress” signboard when conducting any repair or maintenance work.
Recommendations:
To prevent similar occurrences, I suggest the following:
1. A refresher course on safety procedures should be conducted.
2. Details of the person/s to contact in the event of an emergency should be displayed.

Besides, machines are run and controlled by pre- programmed computer automation. This ensures high degree of accuracy and reduces wastage and prevents human causalities. Last but not least, The research and use of renewable energy is being exploited very well by the mechanical industry. Some popular renewable energy such as: solar energy, wind energy, wave energy,.. are appearing in many parts of the world. This greatly reduces the use of fossil fuels. In the future, the mechanical industry will still develop more green energy, it will contribute to creating a less polluted earth.
