Triage is a contentious issue in medicine, with many nuances and ethical questions for all those concerned, including patients and practitioners. Patients in need of treatment are the ones who are most affected by this process, and are also the ones who suffer the most. Medical treatment in the emergency room can have negative effects, such as delays in delivering care, compromises in privacy and security, inadequate physician-patient contact, failure to deliver necessary care entirely, or even having to choose which lives to save where not all can be saved.

Second, as disaster strikes, hospitals must lock their doors long before emergency-level capacity is reached, relocating resources and diverting prospective patients to pre-designated secondary healthcare locations.
