         <description><![CDATA[<div>There are three different models currently being trialled in Sussex across 13 eHubs, covering 1 million patients.</div><ol><li>eHub First Model: An online consultation will be reviewed by the eHub team first, where a team of clinical and administrative staff will direct the consultation to an appropriate outcome.</li><li>Practice First Model: An online consultation is reviewed by the individual practice first and then filtered to the eHub team as appropriate, along with other administration and/or clinical tasks depending on capacity in the individual practices.</li><li>Administration Hub Model: A team of administrators across a PCN are linked up digitally with a group of practices to support signposting and patient services remotely in order to improve patient access and support practice resilience. Their role is to process and complete tasks for each practice the hub is supporting. Precisely which tasks is determined by the specific needs of each practice and patients on the day.</li></ol>]]></description>

         <title>What is Community Pharmacist Consultation Service (CPCS)?</title>

         <description><![CDATA[<div>NHS CPCS is a Community Pharmacist led clinical service. Community Pharmacy has been managing referrals from NHS 111 since 2019. Following a national pilot in November 2020, the NHS CPCS pathway is now national, and GP practices are able to send referrals for minor illness conditions.&nbsp;</div><div><br></div><div>Practices can screen patients using their existing screening capabilities, streaming protocols they have in place or the inbuilt screening questions in the electronic referral to the Community Pharmacy. When the referral is received, the Pharmacist will carry out a consultation with the patient which can include advice or purchase of an over the counter (OTC) medicine.</div><div><br></div><div>If following the consultation, the patient needs to be escalated or referred to another healthcare setting then the pharmacist can arrange this. A consultation summary is sent to the patient’s registered practice.&nbsp;</div>]]></description>

         <title>What is Proxy Access for Repeat Medication Ordering in Care Homes?</title>

         <description><![CDATA[<div>Proxy access for repeat medication ordering in care homes was developed to allow selected care home to access and manage parts of their residents GP online services account. The ‘proxy’ (care home staff member) is given their own online access account (rather than using the patient’s login details).&nbsp;</div><div><br></div><div>GP practices can set up proxy access to order repeat medications only, meaning that care home staff can’t see anything else in the record. If, however, a resident does not want care home staff to have proxy access to <em>any</em> part of their record, including repeat medication, the care home must continue using a paper-based process.</div>]]></description>
