         <description><![CDATA[<p>A program that does everything you need for medical history purposes. Communicate with the physician and look up the results of the test. That part is not too new. The newest part is filling out the new patient information that goes directly into the chart, and makes the first visit at the MD offices faster by just signing in. Also, a program where your actual visit is documented in a progress note and filed automatically. </p>]]></description>

         <description><![CDATA[<p>In my opinion, this is a great idea. Currently, different areas have different portals that do not communicate. These portals are great for reviewing progress notes, speaking to the nurses, and referring to tests that were ordered. I have 3 different portals I use. One for my PCP, rheumatologist, and MyChart for my preferred hospital. This is not counting my son's portals. My MDs have access to MyChart, but if I am ever hospitalized, the hospital does not have access to the other portals. Unless they are part of the hospital. The fact that I have different portals tells you they are not partners. </p><p><br></p><p>One of the big issues nowadays is overmedication, due to having more than one MD write the same prescriptions. Left hand does not know what the right hand is doing. </p><p><br></p><p>If everyone uses the same portal, communication between MDs is more accurate, and they can see current medications.  I am not only talking about opioids or any other controlled medications, which is a big problem.  I have personally seen patients taking multiple medications of the same classification or medications that can react to each other. For example, multiple classifications of blood pressure medications, and wonder why they get admitted for multiple falls related to either regular hypotension or orthostatic hypotension.</p><p><br></p><p>When you ask the patient why they are taking those medications, they pretty much have the same answer. "I don't know, the doctor said I should take it."</p><p><br></p><p>Also, it can be cost-effective. Patients may not know what recent test they have had, or how recent. If MDs are all on the same portal with one medical record, they can see and prevent ordering a repeat test, which can decrease the cost for patients. PCP ordered an MRI of the brain for headaches and referred to a Neurologist. The Neurologist can use that MRI result instead of ordering one for "their records." Which decreased cost because it may either be too soon for insurance to pay, and if you are uninsured they you do not have to pay for the test twice if it's so soon after the first one. </p>]]></description>

         <description><![CDATA[<ol><li><p>Relator- I like to achieve a goal with a team that I also consider close friends. We understand how each other works without hurting anybody's pride. </p><p><br></p></li><li><p>Competition- Having the best product out there is also considered being first in my book. </p><p><br></p></li><li><p>Achiever-  This comes in handy when you keep hitting walls and feel like giving up. Stamina and determination will assist in getting to the finish line.</p><p><br></p></li><li><p>Consistency- A program like this does not discriminate. Everyone will be treated the same way. Having something like this assists in making it easier for everyone to follow the rules and procedures of the board and patient care. </p><p><br></p></li><li><p>Restorative- As mentioned in my rationale, there are a few main problems that this program will resolve. In turn assist in many minor problems along the way. </p></li></ol>]]></description>

They can approach the problem in their different strengths to achieve the goal in the best way possible.</p><p><br/></p><ol><li><p>Carter (Communicator)- I am a very wordy person. He can assist in the best and most effective way to explain steps, situations, topics, and goals.</p><p><br/></p></li><li><p>Chelsea (Command)- She can help with getting a clear and decisive thought process after brainstorming ideas. </p><p><br/></p></li><li><p>David (Deliberative)- He can assist with reducing risk and problems through this process.</p><p><br/></p></li><li><p>Sheila (Strategic)- She can help with different ways that can be easier and successful in reaching the goal.</p><p><br/></p></li><li><p>William (Woo)- The ability to connect with all kinds of people and build rapport is one of my weaknesses.
