TEAM POSITION: Thoughts & Perspective

Peripheral intravenous (PIV) catheter placement failure rates, in hospitals, are as high as 50%. Replacing PIV's can be costly and can increase risk of infection. Factors complicating PIV failure rates are diseased vessels due to drug abuse, co-morbid conditions, and intravenous medications that cause vessel injury. (Helm, Klausner, Klemperer, Flint, & Huang, 2015).
In an attempt to improve patient and clinician satisfaction while reducing costs associated with PIV insertion and use this team proposes a trial of the EchoNous ultrasound for PIV placement.
The EchoNous allows clinicians starting PIV's to distinguish healthy veins, for cannulation, at significant depth. Cannulation of healthy veins at an appropriate depth may reduce intravenous associated infusion complications (Gottlieb, Sundaram, Holladay, & Nakitende, 2017). The EchoNous is FDA approved and may be used on adults and children. Additionally, the ultrasound technology may be used for peripherally inserted central line and midline catheters (Businesswire, 2018, EchoNous, 2018).
While drawbacks of the EchoNous ultrasound PIV placement technology are initial cost and costs associated with clinician education our team believes we can show greater benefit to loss with a single hospital unit pilot trial.

Image from: http://www.sagewaysconsulting.com/the-spark-of-insight/shutterstock_157339613-spark-lightbulb/
