Visual skin assessment is dependent on surface alterations in the skin, by the time pressure ulcers(PU) are noticeable, the underlying tissue may have already suffered damage that is not immediately apparent, and PU prevention measures may be ineffective (Tobiano et al., 2023).The Provizio sub-epidermal moisture (SEM) scanner is an innovative hand-held portable device used for the early detection of a forming pressure ulcer(PU) at the heels and sacrum. It is the only tool currently available that supports the clinician's PU risk assessment. The SEM Scanner is aimed at a particular phase of the PU cascade, during which a window of opportunity exists for the identification of a localized alteration in the biocapacitance characteristic of a tissue area that is susceptible. Such a shift would point to potentially reversible inflammatory microdamage (Lustig et al., 2022).

The SEM scanner identifies micro dermal changes before it becomes macroscopic and widespread. It is a valuable risk assessment tool for early detection and intervention (Lustig et al., 2022). Microscopic identification of changes lead to a more targeted preventative care. According to Padula et al.(2020), SEM has the potential to improve patient outcomes and improve the efficiency of hospitalization cost. A SEM scanner is estimated to prevent roughly 7 HAPI-related deaths and reduce HAPI-related re-hospitalization by roughly 206 bed-days for every 1000 admissions.

The SEM scanner can only be used to escalate intervention but not to de-escalate intervention. Usage of the device is restricted to prescription use only and can not be used for detecting or diagnosing PUs. The device can also be a source of infection if not properly cleaned and disinfected between patient use (FDA, 2017). The locations for usage are restricted to the heels and sacrum, Brunetti et al.(2023) reported that although SEM assessment technology has been proven to work with bench, computational, and tissue phantom models, there hasn't been any soft tissue validation.

I believe this is a revolutionary technology in its early stages that can reduce PU incidences if properly utilized. According to Brunetti et al.(2023), early detection is key to providing effective PU interventions but at the moment, subjective tissue assessments like VSA are the gold standards used to identify PUs, this makes it impossible to intervene before there is noticeable tissue damage. Solutions that can identify early tissue damage before it manifests visually or tactilely are still lacking. SEM measurement-based assessments offer a reliable and objective means of identifying pressure ulcers (PUs) early on, which can help prevent PUs manifestation in patients at high-risk anatomical locations.

Physical contact between the disposable part of the SEM scanner and specific points of a patient's heel and sacrum are currently required but with positive results, I am of the opinion the technology can evolve to a point where broader scans without physical contact can be done with the click of a button.
