The poem appears to take place in Ferrara (Northern Italy) and is narrated in a dramatic monologue by Duke Alphonso II. The "Last Duchess" he speaks of is Lucrezia de' Medici.
Lucrezia had died at the age of 17 (only two years after marrying Alphonso) of suspicious circumstances, and many say she was poisoned.

In the poem, Duke Alphonso II is speaking to a messenger/servant about his latest wife, which had died. He shows the servant a painting and suggests he was angry at her for not being overly grateful of her new title, and that she was easily pleased by other men.
The Duke is supposedly marrying yet another woman this day, and that is who the servant apparently belongs to.
