Make sure that you are very specific about what particular learning component you would like to place on the blended online and offline learning platform.

Areas for Improvement: You mentioned two Key Performance Indicators (KPIs). Make sure that before you implement your TAP, you are able to empirically measure the current state of "motivation" and 'critical thinking skills" of your target audience. Once you have this information, you can then compare these results (BEFORE) to what happens beyond the completion of your TAP (AFTER). Doing this will make the impact of your TAP much more convincing.

Areas for Improvement: You were very specific about accomplishing 95% improvement as indicators for your two Key Performance Indicators (KPIs). Make sure that before you implement your TAP, that you are able to undertake a pre-test, measuring your students' levels BEFORE you implement your TAP. And then, AFTER you implement your TAP, measure your two KPIs in a post-test. Doing this will make the impact of your TAP much more compelling.

I strongly suggest that you and your team veer away from "deficit" approaches in building culture and instead adopt positive approaches.

Areas for Improvement:
I have two suggestions to make your TAP truly transformational:
(1) As a team, it may be a good idea to implement a small-scale transformational cultural change in your school (through your project). Why not adapt an innovative practice, where you try to catch the students doing something good (not something bad) and then reward and highlight positive behavior. One specific way of doing this could be organising a team of students to be "Good Behavior Wardens" whose role is to catch students doing good practices in school. During the timeline of this project, a competition can be undertaken to reward students (and classrooms) who have obtained the highest citations of "good behavior".
(2) As a team, create an online platform (Padlet, for example) where everyone involved can track and monitor students who are participating in this initiative. Identify the best behaved students and duly reward them at the end of the program. In this online platform, the team can also identify the truly recalcitrant students and then devise collaborative plans to address these cases. Areas for Improvement:   As an initial project, it may be a good idea to focus on one or two aspects in the school. My suggestion is one that deals directly with teaching and learning. For example, you can start a small-scale Flipped Classroom mode for one of the senior secondary classes with a specific project (one for this particular TAP that you are doing) that requires them, for example, to do a "paperless" assignment. A paperless assignment that I assign to my students (with great success) is asking them to design a Pinterest Board (as a group) focusing on creating an online resource for a particular component of the curriculum. You can then measure  whether their enthusiasm does improve in paperless, technology-intensive environments. Focusing on a limited set of initiatives (for purposes of this program) will make your TAP more manageable. (2) If I may suggest, teachers who choose to use the room should agree on some sort of covenant (or contract). 1). I think I will use the word "Teachers' Playroom". it sounds friendlier, simpler and fun.
2). Definitely I will connect with ICT, since we are developing an integration school system which will be used web based and Apps on smart phone. I am planning to put some LED TV on it.
3). Having an agreement is excellent idea. I have asked the teachers to start making a blog for their portofolio so if they have made a project or proposed an ideas they must put it on their blog. They just have to send me the link.
4). Some rewards have been prepared for those who will be productive. Material on non material.
5). I am starting to give a challenge to my teachers, what can they do for making school is a fun place to learn and work.

1.       Some teachers are interested to join, but don’t know how to started.
2.       Not interested.
3.       Just ignore it
 
Here are the reason why most of teacher doesn’t want to join :
1.       The workload of teacher is overwhelmed with more or less 30 periods teaching hours and other responsibilities in some school activities committee.
2.       Change mindset cannot be done in a short period of time. This programme is launched in the mid of academic year ongoing where teachers had been set their own goal.
3.       Learning environment doesn’t exist in our school where teachers are individualistic and fell threated to share
4.       The leadership traits example from our principal is not supported teacher to develop themselves. 
 
But I still confidence that Community of Learning is an excellent programme to transformed my school to become a better workplace and create the innovation teaching learning activities ahead.

Contrary to my assumptions, almost everybody, to be exact 29 respondents, have a score between 40 to 50, and 3 people with the score of 30 to 40. This means that I have a great team already, which is in contrast of my assumptions. It is likely that i that some respondents gave the ideal answer expected from them not the reality the real conditions). I've also found out that about 3 respondents got help in filling out the questionnaire, two of them are part timers who did not finish their elementary education and whose contract is not renewed (their contract ended in March). With this finding, I would like to have your suggestion whether it is still relevant to focus on team work for my project or should I just focus on the healthy school project and introducing it through the web? It is good to know that you have modified your TAP to something that you feel is much more important and contextually-relevant. The justification that you have presented is compelling. The theme of wellbeing and bullying is a very serious issue -- this is a relevant point to explore. Overall, your proposed project is very promising.
Suggestions/Comments
You need to make sure that your "stamp" in the project becomes clear. In your presentation, an impression (this is most probably a mistaken one) emerges where the project seems to be an existing school initiative and then you simply "ride on" the project.
