         <description><![CDATA[<p>I am not sure of all my strengths and weaknesses in Blood banks but I can say I am confident when it comes to doing a ABO and Rh typing, I understand the importances of the antibody panels, I mostly understand all the transfusion reactions and how they occur but I know when I begin learning about it again I'll understand it more. My weaknesses would be actually performing an antibody panel along with trouble shooting, elutes, warm and cold antibodies, and learning more about how to process the different units. </p>]]></description>

I work alone on third shift so at times I try to do way too much but I get very discouraged being unable to do it all. I am also still a little panicky when it comes to the immediate response to an unexpected emergency. Sometimes they are expected and I have everything all planned but when I get no notification besides a printed order for an emergency I get very shaken. Its important to me to have a plan and stick to it so when things don't go as I expect it is hard for me. </p>]]></description>

Once I got comfortable with the panels, I actually really enjoyed working on them. Again this was back in school and clinicals but have not worked in blood bank and solve a panel in real life. </p>]]></description>

I sometimes stumble on how to do things like DAT's etc. I can look it up and read the theories and get the samples done it just takes me longer because I usually have to reacquaint myself with how things are done. Some hospitals I have worked at only have blood bank via tube method, no analyzers. My new location will use an analyzer, that is also a weakness of mine but through careful study and practice I will get it.</p>]]></description>

         <description><![CDATA[<p>I would say that my strengths for Blood Bank is in the hands on portion: Type and screens, DAT/IAT's, Antigen Testing. Working in the department has helped me get a bit comfortable with the more common day to day aspects. In saying that, it leads to my weakness. In my current position, I am placed more in core lab over blood bank, only really getting to actually be in the department when I am on night shifts or the few time I'm scheduled on day shifts (outside of weekend day's where typically only one tech is scheduled for the lab). This leads me to not having been able to get comfortable with KB stains/rosette testing,  Antibody panels &amp; identification, and the troubleshooting for it as I did not get a lot of hands on practice during clinicals , leaving most of my experiences coming from my MLT class. Another weakness is remembering the quirks of the antibodies (ex. cold antibodies, antibodies effected by enzyme panels, etc.) as it is not information I consistently have to utilize in my current position. </p>]]></description>

In my hospital we do not deal with these much and most of the time its as easy as getting the Dr. to sign the conditional release form but in crunch time I panic in my mind but rest assured the blood will not be delayed on my part. I try to be as fast as I can while also not making mistakes, but in blood bank I am especially careful because the outcome can be very detrimental to the patient if I'm not. It is very important to me that I deliver the best quality of work possible for the patient's sake. I am an over-thinker in general though, so maybe I sometimes overthink things that I shouldn't. Specifically, MTPs really stress me out because it's a highly emergent situation so you don't really have time to overthink. It just scares me because the more rushed I am, the more likely I am to mess up, and like I said, that can be fatal for the patient when it comes to Blood Bank. </p>]]></description>

         <description><![CDATA[<p>My blood bank experience is confined to a class and a two-week clinical, however, during that time, I passed the class with a high B and I didn't get admonished at the clinical.  With enough practice, I was able to do well with physically performing tests and completing antibody panels (and they were intimidating at first).  As I haven't done any blood bank work since attaining my MLT degree, I'm rusty but I feel that a lot of the knowledge will return through this class.</p>]]></description>

         <description><![CDATA[<p>As I haven't done any blood bank work outside of my MLT blood bank class and two-week clinical, I'm rusty regarding the details of blood bank work.  In particular, remembering the temperature, shelf life and all the other details about the different transfusion products was tricky when it came to test time.  Completing antibody panels in lab took a lot of practice.  Interpreting agglutination tests in lab were pretty stressful when the agglutination was weak/mixed. </p>]]></description>

         <description><![CDATA[<p>One weakness that I still have with blood bank are when it comes to performing rule-outs on antibody panels. I still at times have to really concentrate on what I'm ruling out and what cell will need to be positive and what will need to be negative. I just get confused easily when it comes to that step in working up an antibody ID. Another weakness that I have with blood bank are DATs. I understand the concept of them, and while working I've performed many, but there was this one time I was performing a DAT, and I called it negative when in actuality it was positive. I must've shaken the tube too hard or something. So that incident has caused me to be warry of DATs and if I'm truly seeing a negative reaction, or just a really weak one. </p>]]></description>

I would read through the procedure some nights and I would tell myself "I will be in deep trouble if I get one" I am more of a visual, hands-on learner. I need to go through it a few times before getting the hang of it. Especially something that important. I will also say that another is having a MTP and them having an antibody. OMG! I like to make sure that all of the details are finished and line up but there is often such a push from the clinical care team (RNs and Drs) that you feel like you can't take the time to make sure that everything is right. I dread feeling rushed and anxious in those situations. I've been a blood banker for 3 years and still sometimes get flustered under pressure and have made it my goal going forward to always have a level head and to not let people like that get to me - patient safety always comes first. </p>]]></description>

As soon as I had a feeling what the antibody might be (typically after screen+panel, potentially as im using extra cells to rule in/rule out) i'm already grabbing units for antigen typing and usually by the time the workup is done i'm already on to full crossmatching compatible units. My turnaround times for antibody IDs were always quite good.</p>]]></description>

Such as 3x antibody patients at once, one with a positive DAT and needing an elution while doctors are calling asking for blood....Or multiple MTPs simultaneously and scrambling to thaw and label product, and handle all the paperwork. It can get extremely overwhelming and feel as if I am drowning.</p>]]></description>

My clinical rotation for bloodbank was subpar, and I never had the opportunity during those clinicals to do any emergency releases, MTP's, or specialized testing. My lack of good clinical experience makes it harder for me to fully grasp some of the concepts, mainly because I don't understand exactly why the test is being done, or what exactly the results mean. DAT's and IAT's confuse me as well as crossmatching. Sometimes I mix up the antibodies and antigens because there's so many! I would love to become more confident in my abilities to work the bench.</p>]]></description>

Ruling out multiple antibodies can feel overwhelming, and I worry about missing something important. I also do not yet feel confident selecting the appropriate platelets for patients. Since blood bank work directly affects patient safety, this department feels intimidating to me. I understand that giving the wrong blood type or incompatible blood is life-threatening, which makes this area stressful. Even though blood bank is scary for me, I want to continue learning so I can become more confident and stronger in this department. Don’t get me wrong the ABO/RH I am comfortable with it’s just everything else. I’m not then when it comes to the genotype and phenotype scared of that too.</p>]]></description>
