         <description><![CDATA[<div>1.&nbsp; &nbsp; &nbsp;Superintendent, Treasurer, or representative must attend March and May LERC board of trustee meetings</div><div>2.&nbsp; &nbsp; &nbsp;Superintendent, Treasurer, or representative attend 4 of 5 LERC board of trustee meetings</div><div>3.&nbsp; &nbsp; &nbsp;Hold and attend district meeting with Superintendent, Treasurer, wellness champions, and Findley to discuss wellness offerings and district plan by 10/31 of each year</div><div>4.&nbsp; &nbsp; &nbsp;Submit annual district wellness action plan by 10/31 of each year and share this with internal stakeholders&nbsp;</div><div>5.&nbsp; &nbsp; &nbsp;Pay 11 out of 12 district premiums on time by the 10<sup>th</sup> of each month. Any premium holiday months will be considered an on time payment.&nbsp;</div><div>6.&nbsp; &nbsp; &nbsp;Complete School Health Index assessment (once every 3 years) by 04/30</div><div>7.&nbsp; &nbsp; &nbsp;Formal adoption of district health and welfare philosophy statement, (reviewed and/or revised annually) by 12/31 of each year</div><div>8.&nbsp; &nbsp; &nbsp;75% participation in health screenings and health assessment (benefits enrolled employees)</div><div>9.&nbsp; &nbsp; &nbsp;Promote usage of Medical Mutual MyCare Compare tool</div><div>10.&nbsp; Adopt standardized benefit communications and distribute to all benefit eligible employees</div><div><strong>*The above 10 items are subject to a 12-month measurement period of October – September each year.
