I can infer that he has a severe case of PTSD. Annabelle's grandpa however, believes that shell shock and fear couldn't even change a person that bad. I believe this is because of something horrible he must have experienced during the war. Based on the fact that he hurt his hand, he must have been badly injured. This could be one reason his PTSD is so severe. Some other reasons could be he shot people badly or her saw people die in traumitizing ways.
