Postpartum hemorrhage is the leading cause of maternal mortality in the U.S. with uterine atony, or the failure of the uterus to properly contract after birth, as the most prevalent cause of postpartum hemorrhage. (D'Alton et al., 2021). When uterine atony occurs following delivery, initial management includes uterine massage and administration of uterotonic medications to promote the contraction of the uterus. If these measures are not effective, various devices can be used to help control bleeding (D'Alton et al., 2021). Prior to FDA approval of the Jada System, intrauterine devices to control hemorrhage focused on tamponade methods where a balloon was placed into the uterus and inflated in order to compress the bleeding vessels. These methods have a success rate of 85.8% but requires 12-24 hours of indwelling time and pose risks including expulsion of the device from the uterus, lacerations to the cervix or vagina, uterine perforation, and infection (D'Alton et al., 2021). The Jada System is unique and novel because rather than employing a tamponade method, the mechanism of action is that of vacuum suction. The device is introduced to the uterus through the cervix and connected to low wall suction in order to compress the uterus from the inside (D'Alton et al., 2021). Various trials have proven the efficacy of the device with 94-99% success rates (D'Alton et al., 2021).

Some contraindications to use of the Jada exist including untreated uterine rupture, unresolved uterine inversion, current cervical cancer, known uterine anomaly, or current purulent infection of the vagina, cervix, or uterus. Additionally, the device cannot be used unless the cervix is or can be dilated to at least 3 cm as this is needed for safe placement of the Jada. Efficacy has not be proven for the use of Jada following a birth before 34 weeks gestation, with placenta accreta, or in patients with clotting disorders. (D'Alton et al., 2021).

This device has been proven to be both safe and effective at managing postpartum hemorrhage when conservative management with uterine massage and uterotonic medications is unsuccessful. Various clinical trials have been performed which validate the efficacy and support the use of this device in the clinical setting. Jada is minimally invasive and poses low risk for complications. Use of this device is highly effective at preventing devastating postpartum hemorrhage which if not properly managed can lead to hysterectomy or maternal death.
