A man is rushed into the hospital with severe injuries after a massive car accident. He arrives at the emergency room as the nurses and doctors stand by, ready to admit and treat this patient. The staff quickly assesses and begins to treat this patient for his internal bleeding. Family is contacted and arrives at the hospital. The doctor states that he is losing blood fast and may need a blood transfusion. Family notes that this patient is a Jehovah's Witness, pulls out an advance directive stating that he refuses any sort of blood transfusion.
This situation is common all around the world, where the ethical issue of administering blood transfusions come into play. What is the nurse's role when taking care of these patients?

Nurses are already involved in knowing their role for patients who decline blood transfusions. These are some recommendations on how to sustain and strengthen these roles:

1) Nurses should be encouraged to learn more about the general religious beliefs of patients who refuse blood transfusions.

If more nurses understand understand why patients refuse blood transfusions, they are able to be more prepared to take care of these types of patients and act as a educator for other medical staff.

2) Seek feedback from other staff members if "moral distress" occurs.

Nurses who may not agree with the decision of their patients and families may need to talk to others about their feelings to avoid countertransference. If left unchecked, these feelings may have an effect on how the nurse delivers their care.

3) Continue in encouraging patients for advance directives that include the refusal for blood transfusions, and documenting them appropriately.

Nurses further strengthen their role as patient advocate if they continue to ask or encourage patients for advance directive that legally document their wishes on future care.
