COORDINATION WITH ADULT AGENCIES – Review

Connecting students with outside agencies that serve adults with disabilities helps "create a bridge" between high school and adult life, allowing the student to build community relationships before leaving high school.

When the IEP is developing the Transition plan the following items must be considered when coordinating with an outside agency:

·         There must be documentation that outside agency was considered.

·         If appropriate, is there evidence that a representative of any outside agency was invited to the IEP Team meeting with the prior consent of the parent or student who has reached the age of majority?

·         If agency involvement was needed: 

o    Written consent to invite an agency representative is in the file. 

o     Evidence the agency was invited is in the file; and

o    Written consent is dated prior to the agency invitation for each IEP

·         An adult agency representative is only required to be invited to the IEP meeting when that agency is likely to provide and/or pay for transition services during the current IEP.
